- If you have insurance, please have your insurance information ready for the vaccination clinic. Please bring your insurance card, or insurance number printed.
- If you do not have insurance, you can find the prices of the vaccines below. Safeway/Albertsons cannot serve patients 18 years old with Apple Care insurance. Patients with Kaiser Permanente insurance will only have the flu vaccine covered. If you cannot make this clinic, or if you are 18 and younger without insurance or with Apple Care insurance, you can receive these vaccines or tests in pharmacies and clinics around the Bothell campus.
- Please fill out all the information on the appointment website as early as possible before your appointment. Students 17 years and younger should have their parents sign the appointment information before the event.
- When you are filling out the appointment information, if you do not have an address and/or phone number in the United States yet, please use the Immunity Verification Program’s address at 4060 E. Stevens Way Northeast Campus Box 354410, Seattle, WA 98195 and 206-616-4672.
- You will be able to receive vaccines that help you fulfill your UW general immunization requirements that day, so please make sure you have your student ID number ready and are able to take photos on your device.
- There will be another vaccination clinic in Bothell on October 8th. Please note: If you receive any live vaccine, you may need to wait at least 28 days or more for another vaccine. If you receive an MMR vaccine, you will need to wait 28 days before receiving a second MMR vaccine. You can receive the Meningitis ACWY vaccine and MMR vaccine at the same time.
It is strongly recommended that you make your appointment in advance. Walk-in appointments may also be available, but cannot be guaranteed. Vaccine availability is subject to change.
Below is the list for vaccinations if a patient does not have insurance and wishes to pay out-of-pocket. Please call your insurance ahead of time to check if you will have coverage. Please make sure you have both sides of your insurance card. We are looking for the pharmacy/prescription information.

The weight line is only required if you are under 65 pounds (29.5 kg). It is not required to put in your weight if you are over 65 pounds. This is field is for parents signing their small children up for a vaccine.
If you have a personal care physician, you can write their information down here. If not, you do not need to do so.

If you are 18 or older, you can sign for yourself. If you are younger than 18, please have your parent sign your form for you.
You will be sent a reminder email as well.
They will also send you an email confirming your appointment, as well as a few reminder emails. DO NOT forget your appointment!
