The Health Sciences Immunization Program (HSIP) ensures students enrolled in health science academic programs comply with the requirements detailed in the University’s affiliation agreements with clinical and practicum training sites. These affiliation agreements require students to complete and document immunizations and tuberculosis (TB) screening as recommended by the Centers for Disease Control and Prevention (CDC) for healthcare personnel. The requirements reduce the risk of exposure to communicable diseases for students and their patients/clients. Students are required to comply with updated CDC and HSIP requirements upon enrollment and continue to meet ongoing requirements until completion of their degree program, even if those requirements change during a student’s enrollment. It is the student’s responsibility to meet any requirements of a training site that may differ from those requested by HSIP. Students under 22 years old must also complete the Meningitis ACWY vaccine requirement for the UW General Immunity Verification Program.
Due to the nature of health science training programs, exemptions are granted in alignment with CDC recommendations for documented medical conditions for which there is a vaccine contraindication, HSIP policy, and University policy and practice only. Please refer to the Vaccine Exemptions section below.
HSIP Phone Consultation: If you would like to talk directly to an HSIP staff member regarding your requirements, please schedule a phone consultation by using our Microsoft Outlook Booking page. Please note: Phone consultation appointments need to be made 24 hours in advance; HSIP does not offer same-day consultation.
Knowingly furnishing false information to any University official violates the Student Conduct Code under WAC 478-121-110, and will be reported to the office of Community Standards & Student Conduct.
For a summarized version of your HSIP requirements, print the HSIP Requirements Checklist to bring to your provider. You can also review the full requirement description in the Immunization Requirements section below. Please wait to make a Med+Proctor account until your program has given you the instructions to do so.

- UW Tacoma’s Nursing program and UW Bothell’s Nursing program do not participate in HSIP.
When students are entered into an HSIP-participating program degree code, they will have an automated registration hold. The initial hold is only there to make sure the student creates their Med+Proctor account once it’s time to do so, and does not mean your requirements are late. Once your programs and HSIP are ready for incoming students to start creating Med+Proctor accounts, Med+Proctor and/or your program will send students an invitation. Once the student completes the Childhood Immunization attestation in Med+Proctor, their initial hold will be lifted.
Please do not create a Med+Proctor account before you receive an invitation to do so, as it will cause an issue with your account settings. If you log into Med+Proctor and you do not see the Childhood Attestation, the Tuberculosis Risk Assessment, or the payment screen after creating your account from the invitation, please alert HSIP at myshots@uw.edu.
Beginning with incoming Autumn 2025 students, HSIP plans to transfer current students, who are not graduating by Summer 2026, into Med+Proctor. This will begin in February 2026. Please keep an eye on your email for when you need to access Med+Proctor to make your accounts.
HSIP contracts with a vendor, Med+Proctor, to track immunization and TB testing compliance for the incoming programs. Each health science school or program sends instructions to accepted students for creating their vendor account, including deadlines for meeting HSIP requirements. Some programs require additional accounts for background checks and drug screenings which are not related to the HSIP accounts. There is a one-time fee to set up the vendor account paid directly to the vendor when creating the account, which will be listed in the instructions email sent by your programs. Please do not make an account before your program sends you instructions to do so, as this can affect your account.
Once you enter a participating health science school or program, you will pay the HSIP fee to cover the cost of administering the Health Sciences Immunization Program. The amount of the fee is subject to change on a yearly basis and is attached to your tuition account for as long as you remain in your UW health science school or program.
Read the HSIP Annual Fee webpage for specific details about how the health fee was calculated for the current school year.
Please note that the health fee is not health insurance. More information about getting health insurance coverage can be found on the Husky Health Center website.
We recommend students learn what health services are offered at the student health center on their “home” campus and in the community. For students who need initial or ongoing services in the WWAMI region (Washington, Wyoming, Alaska, Montana, and Idaho), please review our Locations page.
By accepting enrollment into a program/school that requires participation in the UW Health Sciences Immunization Program (HSIP), students agree to meet immunization and TB screening requirements in accordance with CDC guidelines for healthcare personnel and as outlined by HSIP. Due to the nature of health sciences training programs, exemptions are granted only in alignment with CDC recommendations for documented medical conditions for which there is a vaccine contraindication, HSIP policy, and University policy and practice.
HSIP follows the Centers for Disease Control and Prevention’s (CDC’s) Guidelines for Healthcare Workers, which may change during a student’s enrollment. Students are required to comply with updated CDC and HSIP requirements upon enrollment and continue to meet ongoing requirements until completion of their degree program, even if those requirements change during a student’s enrollment. It is the student’s responsibility to meet any requirements of a training site that may differ from those requested by HSIP at the time of enrollment.
HSIP may discuss, share, and communicate students’ compliance status and related information with their health sciences school/program and practicum/clinical training sites prior to and during placements. This includes any or all information provided to meet entry requirements and any annual or other required updates. In addition, HSIP may share student immunization status with the University of Washington’s Employee Health Center in the event of an exposure to a bloodborne pathogen.
This agreement must be signed in Med+Proctor.
The due dates are set by your program. You may submit an exemption to HSIP via myshots@uw.edu passed the listed due date however, it will be considered a late request and the designated reviewer may need more time to review the request.

Immunization Requirements
The Childhood Immunization is an attestation confirming the receipt of the DTap (or DTP/DT) and Polio (OPV/IPV) vaccine series. Documentation is not required. On Med+Proctor this questionnaire will populate once you have made an account.
Respond YES to the attestation if you received the usual childhood immunizations for DTap (or DTP/DT) 3-doses and Polio (OPV/IPV) 3-doses.
Respond NO and contact HSIP at myshots@uw.edu if you did NOT receive DTap (or DTP/DT) or Polio (OVP/IPV). HSIP will provide directions for the adult make-up schedule.
Students must satisfy the Tdap requirement by showing proof of EITHER:
- a Tetanus, Diphtheria, & Pertussis (Tdap) vaccination administered in the past 10 years
OR
- Tetanus & Diphtheria (Td) vaccine administered in the past 10 years AND documentation of one Tdap vaccine administered on or after 6/1/2005.
If your submitted Tdap/Td vaccine dose expires while you are a student, you will be required to receive an new Tdap/Td dose. In Med+Proctor, your account will be unverified and given the tag “UW HSIP- Subsequent Tdap/Td Vaccine Dose” 90 days before your Tdap/Td dose’s expiration date. If an updated Tdap/Td dose is not submitted and approved in Med+Proctor 30 days before your Tdap/Td dose’s expiration date, a registration hold will be applied until compliant.
Students must satisfy the MMR requirement by showing proof of EITHER:
- two MMR (Measles-Mumps-Rubella) vaccines that must be taken after the student’s first birthday and taken at least 28 days apart from other live vaccines, such as Varicella.
- All MMR doses must be administered no sooner than 4 days before their first birthday. If the dose was 5 days or more before a student’s first birthday, we cannot accept it.
OR
- a positive Measles (Rubeola), Mumps, and Rubella IgG titers, which are blood tests to measure immunity.
To fulfill the requirements, MMR vaccines and/or titers must meet the following conditions:
- MMR vaccines must be taken no earlier than 4 days before the student’s 1st birthday.
- MMR vaccines must be taken no earlier than 28 days between all live vaccines, such as MMR or Varicella. MMR and Varicella vaccines taken on the same day is acceptable.
- MMR titers must be at least 28 days after a recent MMR vaccine.
- History of Measles, Mumps, and/or Rubella is not an acceptable document. If you previously had Measles, Mumps, or Rubella, please take a titer test to show your immunity.
- Live vaccines (MMR and Varicella) can interfere with TB screening results, so HSIP recommends completing your TB screening prior to any live vaccines OR completing your TB screening 28 days after a live vaccine. TB screenings completed within 28 days will be rejected and repeat TB testing will be required once it’s been 28 days from your recent live vaccine.
Students must satisfy the Varicella requirement by showing proof of EITHER:
- two Varicella vaccines that must be taken after the student’s first birthday and taken at least 28 days apart from other live vaccines, such as MMR.
OR
- a positive Varicella IgG titer, which is a blood test to measure immunity.
To fulfill the requirements, Varicella vaccines and/or titers must meet the following conditions:
- Varicella vaccines must be taken no earlier than a student’s 1st birthday
- Varicella vaccines must be taken no earlier than 28 days between all live vaccines, such as MMR or Varicella. MMR and Varicella vaccines taken on the same day is acceptable.
- Varicella titers must be at least 28 days after a recent Varicella vaccine.
- History of Varicella is not an acceptable document. If you previously had Varicella, please take a titer test to show your immunity.
Students must satisfy the Hepatitis B requirement by showing proof of BOTH:
- Documentation of 3 Doses Hepatitis B (Engerix-B, Recombivax HB, or other HepB vaccine) OR 2 doses of Heplisav-B
AND
- a positive post-vaccine series Quantitative Hepatitis B Surface Antibody Titer
Step 1: Hepatitis B Vaccine Series
Students should try to obtain their childhood immunization records. If you cannot obtain documentation of a previous vaccine series you do not have proof of your Hepatitis B vaccines, you are required to repeat the vaccine series.
If your vaccine series is in process, submit where you are in the series. New alerts will be created for you to complete the vaccine series and titer. A titer will NOT be accepted until after a complete HepB vaccine series has been submitted.
Acceptable Vaccine Doses and Schedules
Your Hepatitis B vaccine doses need to meet the minimal intervals to be accepted. If you have a dose that does not meet the minimal intervals, then that dose is considered invalid, and you will need to complete a new Hep B dose to replace the invalid dose.
- Energix-B, PreHevbrio, or Recombivax-HB (3-dose series): Doses are given at respectively 0, 1, and 6 months (*minimum dosing intervals are acceptable: dose 1 to dose 2: 4 weeks / dose 2 to dose 3: 8 weeks / dose 1 to dose 3: 16 weeks)
- Twinrix (HepA-HepB 3-dose series): Doses are given at respectively 0, 1, and 6 months (*minimum dosing intervals: dose 1 to dose 2: 4 weeks / dose 2 to dose 3: 5 months).
- Accelerated Twinrix dosing schedule (4 doses; dose 1 to dose 2: 7 days / dose 2 to dose 3: 21-30 days / dose 3 to last dose: 12 months) is accepted. However, students who choose this option should remember that 1 month after the last dose, they will still need to receive the titer to prove immunity.
- Heplisav-B (2-dose series): Doses are given at least 4 weeks apart.
Step 2: Quantitative Hepatitis B Surface Antibody
After completion of a full Hep B vaccine series, you need to complete a Quantitative Hepatitis B Surface Antibody Titer (HBsAb-QN).
We require a positive Hepatitis B titer result in a laboratory report with the quantitative numeric interpretive reference range. An immunization summary or Employee Health titer is not acceptable. A qualitative titer test that only says positive with no reference range is not acceptable. Please note that we do not require a core or Antigen test unless you are a non-responder or have a chronic infection.
Negative Hepatitis B Titer
If your titer is negative or equivocal:
- Submit the negative titer to your immunization portal.
- Receive and submit one additional vaccine dose (Heplisav-B highly preferred) immediately.
- Get a repeat titer 1 month after the booster dose, and submit the report. If it is positive, you have satisfied the requirement.
- If the second titer is negative, you will need to receive the remaining Hepatitis B vaccine series. If you chose the 2 dose series, you will need to complete the remaining 1 dose. If you chose the 3 dose series, you will need to complete the remaining 2 doses.
- Get a final titer 1 month after the last vaccine dose in the second series. If it is positive, you have satisfied the requirement.
- If this last titer is negative, please email HSIP at myshots@uw.edu.
Hepatitis B Non-Responder
An individual is considered a Hepatitis B non-responder once they have completed two documented Hepatitis B vaccine series followed by a negative/equivocal HBsAb titer. Hepatitis B non-responders must complete a Hepatitis B Surface Antigen Titer (HBsAg). If the antigen titer is negative, then your provider needs to complete the HSIP Hepatitis B Non-Responder Counseling Form. Please email myshots@uw.edu to request this form.
For a non-responder, you must complete the following tasks:
- Obtain your most recent Hepatitis B Surface Antigen (HBsAg) titer lab report.
- Please have your provider complete the HSIP Non-Responder Counseling Form.
Once you have completed the form, please submit them directly to HSIP myshots@uw.edu.
Please note that both the student and the program have the responsibility to communicate the Non-Responder status with your practicum site(s), to ensure any adjustments necessary can be made.
Hepatitis B Chronic/Acute/Past Infection
Individuals with a known history of chronic or acute Hepatitis B Infection should complete the following steps; please request forms from the HSIP at myshots@uw.edu. If your antigen titer is positive, then your provider needs to complete the HSIP Acute-Chronic Infection Counseling Form. Please email myshots@uw.edu to request this form.
- Obtain your most recent Hepatitis B Surface Antigen (HBsAg) titer lab report.
- Please have your provider complete the HSIP Acute-Chronic Infection Counseling Form.
Once you have completed the forms, please submit them directly to HSIP at myshots@uw.edu.
Please note that both the student and the program have the responsibility to communicate the Chronic HepB Carrier status with your practicum site(s), to ensure any adjustments necessary can be made.
A baseline individual TB risk assessment must be completed upon entry into your program. The risk assessment is required in addition to the TB Screening requirement and can be useful in interpreting TB test results.
The TB Risk Assessment will be a questionnaire in your Med+Proctor account which will populate once you have made an account. There will be three Yes/No questions which you will need to complete.
Students must satisfy the Tuberculosis (TB) requirement by completing a TB screening from the current calender year. Review the Due Date section for the Acceptable Initial TB Date Range of your program. The TB screening requirement can be completed by EITHER:
- IGRA blood test (either a QuantiFERON Gold or T-Spot lab test)
- Preferred option for all students. Students with a history of receiving the BCG vaccine against TB should get the IGRA test to avoid a false positive PPD skin test.
OR
- TWO separate TB skin tests/PPDs
- First PPD placement and reading 48-72 hours later equals ONE PPD skin test. A SECOND PPD skin test must be placed between 1-3 weeks after the first PPD is read. The HSIP TB PPD Skin Test Form is recommended to ensure all required documentation is included.
- Required: Full name, date each PPD skin test was placed, date each PPD skin test was read, and results documented as a measurement in millimeters.
- “Self-reads” are not acceptable.
Complete Tuberculosis (TB) screening (IGRA or TB PPD skin tests) before OR 28 days after receiving a live virus vaccine (MMR, Varicella). Same day IGRA blood draw and administration of a live vaccine is OK.
Positive TB Screening
If you have a positive TB screening, you are required to complete and submit the following:
- Documentation/proof of positive screening (one of the following):
- Positive IGRA result (either a QuantiFERON Gold or T-Spot lab test) or PPD result (10 mm or greater) OR
- HSIP Positive TB Screening Form completed by a healthcare provider
- Chest x-ray radiology report completed AFTER positive TB screening
- Note: If you are submitting documentation of a completed course of prophylactic antibiotic therapy, including medication name and dates of administration are required.
- Complete the HSIP TB Symptom Survey (TBSS) Form.
Students with a positive TB submission will be required to fulfill the TB Symptom Survey (TBSS) requirement annually December 20. Submit an updated TBSS Form to meet the TBSS subsequent action. You DO NOT need to complete a new TB test or Chest x-ray.
NOTE TO STUDENTS: In the event of either a TB exposure, a practicum site requiring an updated TB screening, or for those who are working in a high-risk area, additional TB screening may be required. Please contact HSIP at myshots@uw.edu to have a new action created to upload any additional TB screenings.
Students in the Medical Laboratory Sciences, Laboratory Medicine, and Pathologists’ Assistant program are required to receive a Color Vision screening through HSIP staff. This will be arranged for you; do not schedule your own color vision screening. If you do have color vision deficiencies, your status will be recorded to your instructors for accessibility and alternate identifiers.
Students must satisfy the Influenza requirement by showing a flu vaccine taken from the upcoming/current flu vaccine season. An annual fall seasonal influenza vaccine is required after August of your entry year and each year thereafter.
- Spring 2026 ABSN admits are required to submit a 2025-2026 Influenza vaccine upon enrollment.
- Summer/Autumn 2026 and Winter/Spring 2027 admits, students are required to provide the 2026-2027 formulation of influenza vaccine, available in August.
Only Influenza vaccines administered after 08/01 of the current flu season will be accepted. Previous flu vaccines are not accepted. All flu vaccines must show the location a student received the vaccine, such as the pharmacy or clinic they received the vaccine.
Exemption requests for the autumn vaccine requirements (influenza) are due at the same time of your program’s exemption due date listed in the Due Date section. You may submit an exemption to HSIP via myshots@uw.edu passed the listed due date however, it will be considered a late request and the designated reviewer may need more time to review the request.
Medical Exemptions
Vaccine medical exemption requests must be submitted by the exemption due date based on your admission quarter into your HSIP participating program. Students with new or worsening health concerns may submit a medical exemption request at any time.
Medical exemption requests are reviewed by the HSIP medical director for medical validity. Additional testing, medical consultation, and/or documentation may be required to receive an exemption.
Instructions for submitting a vaccine medical exemption request: Complete the Health Science Immunization Program (HSIP) Participant Medical Exemption Request for Required Vaccines form and submit it to HSIP at myshots@uw.edu. Do not upload a request into Med+Proctor.
Please note: Antibody titers will be required for MMR, Varicella, and Hepatitis B vaccine exemptions to verify immunity status even if a vaccine exemption is approved. TB screening is not eligible for exemption.
Religious Exemptions
Vaccine religious exemption requests must be submitted by the exemption due date based on your admission quarter into your HSIP participating program. Students may submit a vaccine religious exemption request after the due date; however, programs may not be able to accommodate late requests.
Religious accommodations for required HSIP vaccines must be approved by the student’s school/program.
Instructions for submitting a vaccine religious accommodation request: Complete the Health Science Immunization Program (HSIP) Participant Religious Accommodation Request for Required Vaccines form, Sections #1 to #4, and submit it to HSIP at myshots@uw.edu. Do not upload a request into Med+Proctor. Do not sign Section #6 until the exemption has been approved. The HSIP manager will forward your request to your program’s designated program executive for review.
Please note: Antibody titers will be required for MMR, Varicella, and Hepatitis B vaccine exemptions to verify immunity status even if a vaccine exemption is approved. TB screening is not eligible for exemption.
Non-Religious Declination
Due to the nature of health science training programs, non-religious vaccine declinations are not accommodated by health science programs participating in the Health Science Immunization Program (HSIP). Please contact your program directly if you have questions regarding personal non-religious declinations.
Exception: Students enrolled in HSIP participating School of Public Health MPH programs may request a non-religious vaccine declination; however, accommodations of an approved declination will require students to complete practicum/capstones in a non-healthcare setting. Contact the HSIP at myshots@uw.edu for information.
All required documentation components must be included in your vaccine record submission:
- Full name. If the name on your record does not match your current legal name you will be required to verify your name change through submission of a legal name change document (court issued record, marriage certificate, etc.).
- Name of vaccine administered and full date of administration (mm/day/year).
- Documents must be from a verifiable medical source (medical record, clinic or provider signature/stamp, or certified state immunization registry).
- Handwritten K-12 records signed by parents are not acceptable.
- When submitting a vaccine booklet the following must be included:
- The front page of the booklet with full name must be included in the same file upload as the vaccine documentation.
- Each vaccine dose listed must include a provider signature/initials or facility name.
- Pharmacy printouts, provider/clinic-verified childhood immunization information, and state registry records are all acceptable proof of immunization.
- The military immunization form DD 2766 is acceptable only for immunizations; all titer and lab tests must be a laboratory test result document.
All laboratory reports must have the student’s full name and the specimen date of collection (blood draw date), the test name, and the result clearly stated.
- MMR & Varicella IgG Titers – results may be qualitative (e.g. positive/negative)
- Quantitative Hepatitis B Surface Antibody Titer – results should be quantitative (numeric value). A qualitative result (positive/negative or reactive/nonreactive) is acceptable only when a numeric interpretation reference range is also included (e.g. immunity ≥10 mIU/mL).
- IGRA Blood Test – Acceptable IGRA blood tests are either a QuantiFERON-TB Gold test or a T-Spot test. Qualitative results (positive/negative) accepted. Numeric values preferred but not required.
- The Result Trends document is not accepted since the source of the lab is not included.
If your records are not in English, please make sure your name is in English on your documents. Your records should be easy to read and tell what titers, immunizations, and screenings you received. English translation are highly preferred but not required.
JPEG and PDF file types are preferred. Please make sure your documents are not password-protected, and that your images are clear and readable before uploading. You can upload as many documents as needed into Med+Proctor.
Printing Documents from a Patient Portal
Use the following instructions to pull an acceptable lab report from your MyChart/Paitent Portal. This document should have your full name, test name, collection date, results, and show the source of the record.
The Result Trends documents are not acceptable; this type of report does not include the source of the record and therefore is not verifiable.
If a lab was completed through LabCorp or Quest Diagnostics, you can find the lab report through their sites.
You are considered temporarily compliant if you have some requirements that you are between vaccine doses for, or waiting to complete your Quantitative Hepatitis B Surface Antibody Titer.
Submit each vaccine dose as it is received. In Med+Proctor, your account will be updated with the “UW HSIP- Subsequent (Action)” tag while you are in the process of the vaccine series/titer and a temporary compliant due date. You are considered temporarily compliant while completing the vaccine series/titer as long each new due date is met, and documentation has been submitted and accepted into Med+Proctor.
Students with overdue or rejected items past the due date are considered non-compliant. If you are non-compliant beyond your program’s due date, then a registration block may be applied for the upcoming quarter until you are fully compliant or temporary compliant.
How to Fulfill Your Immunization Requirements
Students should check with their primary care provider for the students’ vaccinations, or any organization where they received vaccinations.
Students can also check with their state or country department of health. For vaccinations taken in Washington state, students can use the website MyIR Mobile. For students who had vaccinations in other states, please find your state’s immunization database on the HSIP Map of State Immunization Information Resources. For students who received your vaccinations outside of the United States, please receive your immunization documents from your primary physician.
Suggested places to check for old immunization records:
- Immunization records from previous clinics or providers
- Pharmacy prescription documents for vaccinations
- Previous employers (employee health records) including military
- Military form DD-2766 are accepted for vaccine records, but not titers. Military personnel must receive titers with valid laboratory results.
- College or other post-secondary institutions (K-12 records that are not provider-verified are not accepted)
- Department of Health or state records
- Provider-verified immunization booklet
- A facility stamp or provider signature is required next to each recorded vaccine dose. Unverified records should be taken to students’ provider or local public health clinic and documented in an official medical record or state immunization registry.
For finding new vaccinations or test screenings, please review our Locations for Vaccinations and Tests page. You can find most vaccines in pharmacies and clinics, and titer or TB screening tests in clinics or laboratories. HSIP students in WWAMI campuses can also use the HSIP Healthcare Resources Sheet to find locations closer to them.
The UW HSIP is not responsible for students’ insurance or covering vaccines or titer tests, although we do recommend having insurance.
If a student is looking for locations on or near the three UW campuses that offer vaccinations or titer tests, the student can look through the following links to find a suitable option. The student should contact clinics directly regarding their sliding scale or financial assistance options, as well as their insurance. When a student requests financial assistance, they should be ready with medical and tax information.
If a student is interested in having insurance but does not know how to get started, the student can make use of Husky Health’s Insurance and Cost explanation, which explains insurance options and how to apply.
To speak with an insurance navigator, someone who can help you get enrolled and answer your questions, students can reach out to the agents on the Washington Health Plan Finder website. They can also find support by enrolling through insurance brokers.
Seattle students can call Husky Health Center at 206-597-5244 to make an appointment with an insurance navigator; Tacoma students can call Tacoma Office of Student Advocacy and Support at 253-692-5934. There are no current options for Bothell students specifically, but students are welcome to research through the WA Health Plan Finder.
If a student has insurance through their job they are planning to leave to study full-time, we recommend doing all and any requirements while you are currently insured.
The University of Washington retains records for 6 years after your last contact with the University. If HSIP no longer has copies of the records, we may still have summary data. To request your records, you should email us at myshots@uw.edu with your full name, date of birth, student ID number and/or NetID, and your record request. We cannot send you records without an email request.
If you have had an HSIP account, you have already paid for access to your documents. Looking for your records from CastleBranch or Med+Proctor? Email us at myshots@uw.edu with your full name, date of birth, student ID number and/or NetID, and your record request.
Med+Proctor Account Tutorial
Once your program and HSIP are ready for incoming HSIP students to start accessing Med+Proctor accounts, Med+Proctor and/or your program will send you an invitation. Please do not access your Med+Proctor account before you receive an invitation to do so, as your account may not be configured as an HSIP account. If you log into Med+Proctor and you do not see the HSIP Agreement, Childhood Attestation, and the Tuberculosis Risk Assessment after accessing your account from the invitation, please alert HSIP at myshots@uw.edu.
Accessing Your Med+Proctor Account
To access your Med+Proctor account, please sign-in through SSO using your UW NetID. It may require Two Factor Authentication (2FA). If you are unable to access your 2FA, please review the UW-IT’s 2FA information.
Med+Proctor Profile
On your first time signing into Med+Proctor, you will need to confirm your date of birth in month/day/year (MM/DD/YEAR) format.

Your first and last name listed in Med+Proctor is the Preferred Name you wish to use that differs from your Student Record Name. If your legal name is your preferred name, then your Student Record Name will be listed instead. Records submitted under your student record name will be accepted even if your preferred name is listed in Med+Proctor. If you submit records under a different name which does not match your preferred name or your student record name, then legal name change documentation will be required.
Agreements & Questionnaires
You will be prompted to agree to the End User License Agreement and the HSIP Agreement. Both agreements are located in the Profile tab of your acccount.

You will also need to complete the Childhood Immunization Attestation and the TB Risk Assessment Questionnaires. Both questionnaires are located in the Questionnaires tab of your acccount.

As a reminder, if you do not see these agreements, please contact at myshots@uw.edu for troubleshooting.
Requirement Tags
Med+Proctor uses tags to flag your account for requirements. You can review the tags listed in your Med+Proctor account through the Status page; you can click on the tag name for more information about the tag and what needs to be submitted. The tag system will be explained more in the Document Review Status section of this tutorial. Initially, your account should have all the following requirement tags listed:
- UW HSIP – Required Childhood Immunization
- UW HSIP – Required Hepatitis B
- UW HSIP – Required MMR
- UW HSIP – Required TB Risk Assessment
- UW HSIP – Required Tdap
- UW HSIP – Required Tuberculosis Screen
- UW HSIP – Required Varicella
- UW HSIP – Required Influenza (You will see this tag in your inital log-in if your admitting quarter is Spring or Winter; Summer Quarter and Fall Quarter admits will not see this tag until 08/15.)

After your initial log-in, the Med+Proctor Status page will become the landing page when logining into Med+Proctor.
HSIP students are to required to pay a one-time fee to Med+Proctor. The fee allows students to have lifetime access to their Med+Proctor account records and have guaranteed 24-48 hours review time for submissions. This fee also allows HSIP to continue using Med+Proctor as our immunization compliance vendor. HSIP students cannot opt-out of the one-time fee. If you log into Med+Proctor and you do not see the payment screen after accessing your account, please alert HSIP at myshots@uw.edu.
If your program’s duration is less than 4 years, the cost of the account is $24. If your program’s duration is 4 years or more, the cost of the account is $29.
You cannot submit documents into Med+Proctor until you have paid the one-time fee to Med+Proctor. If your Med+Proctor account is not configured as an HSIP account, then you can submit records but note that any submitted records will not be reviewed for HSIP requirements. Once your account is configured correctly into an HSIP account, any submitted records will be reviewed in the next 24-48 hours and you will be given HSIP requirement tags. You will need to pay the one-time fee before you can submit records again.
Start submitting records as soon as you have created your Med+Proctor account and reviewed the requirement tags located in the Status page. Your requirement tags in Med+Proctor will be updated as your records are reviewed, so you are aware of what you need to complete next. You can always review the tags in your account by checking the Status page and clicking on the tag name for more information on what is required for submission.
Uploading Documents into Med+Proctor
To make the uploading portal appear, click on Document and then Documents. When submitting, please remember to include the identification page or section of each document; we need to see your full name in all of your submissions. PDF and JPEG file types are preferred. Please note that if you do not press ‘Continue’, your uploads will be lost.

Once you have submitted your first document, you will be taken to the Status page of your Med+Proctor account. If or when you need to submit another document, you can either click on Document (as stated above) or “Submit another document” in the Status page. Both methods will take you to the uploading portal, but the “Submit another document” method is faster.

Document Types
There are several document types in which you can categorize your submissions as. While the document type chosen does not necessarily matter, it is helpful to HSIP and Med+Proctor reviewers to know what to expect when seeing the submission. Below is an explanation of each document type and how they should be used:
- UW HSIP – Vaccine Record: Use this when submitting an immunization summary or a non-Hepatitis B (HepB) vaccine record.
- UW HSIP – Lab Titers: Use this when submitting a MMR and/or Varicella titers.
- UW HSIP – Tuberculosis Results: Use this when submitting your TB screening results.
- UW HSIP – HepB Immunization/Lab Titer: Use this when submitting a record that is specifc to HepB titer(s) and/or vaccine(s).
- If you are submitting an immunization summary that includes HepB vaccines along with other HSIP-required vaccines, then you do not need to use this document type; you do not need to upload the record twice.
- UW HSIP – TB Symptoms Survey Form: Use this when submitting an HSIP TB Symptom Survery (TBSS) Form. This is required for only students who have tested positive for the TB screening.
- UW HSIP – Name Change Document: Use this when submitting a legal name change documentation, if you are submitting a record under a different name from your Preferred Name or Student Record Name.
- UW HSIP – Exemption Form: HSIP uses this document type to upload approved exemptions. Please do not submit your exemption requests into Med+Proctor; email them directly to HSIP at myshots@uw.edu.
Once you have submitted submitted records, Med+Proctor will review your records in the next 24-48 hours. Please monitor your account to see your requirement tags update. If you have requirements that are unfulfilled, you will receive weekly reminder emails from Med+Proctor until your account is verified (compliant).
Med+Proctor Tag System
Med+Proctor uses tags to flag your account for requirements, rejections, and/or subsequent actions. You can review the tags in your account through the Status page; you can click on the tag name for more information about the tag and what needs to be submitted.
Initially, your account will have all of the “UW HSIP – Required [X]” requirement tags, as mentioned in the Initial Log-In section. Once you start submitting records, your account will be updated with either of the following tags:
- UW HSIP- Subsequent [X]: This tag is used when you have a subsequent action to complete for a requirement. For example, if you submitted 1 MMR dose record then you will receive the “UW HSIP- Subsequent MMR Dose #2” tag. This tag lets you know you need to submit a 2nd MMR dose for the MMR requirement.
- UW HSIP- Rejected [X]: This tag is used when you have submitted a dose or record that was rejected.
- UW HSIP- Admin Review [X]: This tag is used when Med+Proctor flags your submission for HSIP’s review.
- You will see the “UW HSIP- Admin Review HepB” tag every time a HepB-related record is submitted. This is because Med+Proctor does not review HepB records, HSIP does. Once HSIP has reviewed your HepB submission, the admin review tag will be removed and your HepB-related tags will be updated.
When you have satisfied an HSIP requirement, all tags associated with that requirement will be removed.
There are some permanent tags which will remain in your account forever. These tags allow Med+Proctor to verify your account once you have fulfilled your other HSIP requirements. Depending on your submissions, you may have any of the following permanent tags in your Med+Proctor account after you have completed your HSIP requirements:
- UW HSIP- HepB Approved (commonly seen permanent tag)
- UW HSIP- Positive TB Screening Approved
- UW HSIP- Approved ME [X]
- UW HSIP- Approved RE [X]
- UW HSIP- HepB Non-Responder Status
- UW HSIP- HepB Carrier Status
How do I know if I am compliant in Med+Proctor?
Once you have completed all of your HSIP requirements and they have been approved in Med+Proctor, your account will be marked as “Verified”. Verified means you are compliant with your requirements at this time. Your verified account should look like this along with only the permanent tag(s) listed as mentioned above.

How do I know if I am temporary compliant in Med+Proctor?
You are considered temporarily compliant if you have completed most of your HSIP requirements but have some that you are between vaccine doses for, or waiting to complete your Quantitative Hepatitis B Surface Antibody Titer. You are eligible for temporary compliance once all of your HSIP requirements have been approved in Med+Proctor and you only have the requirements you need to wait to complete remaining.
Once you are eligible for temporary compliance in Med+Proctor, your account will be verified, the “UW HSIP- Temporary Compliant” tag will be added, and you will be given a Temporary Compliant Due date (the date can be found in your Med+Proctor Immunization Certificate section). At this point, you will stop receiving weekly reminder emails since your account is verified.
The Temporary Compliant Due date is the date of when you should complete your requirement, not when you submit the record. You can submit documentation after this due date has passed.

Once the Temporary Compliant Due Date passes, your Med+Proctor account will automatically become unverified and the “UW HSIP- Temporary Compliant” tag will be replaced with “UW HSIP- Temporary Compliance Ended”. You will start receiving weekly reminder emails again letting you know you need to complete your requirement now.

Upcoming Requirements Process
If your Med+Proctor account was previously verified but you have an upcoming requirement such as:
- Expiring Tdap/Td Vaccine: Your Med+Proctor account will become unverified 90 days before the 10-year expiration date of your Tdap/Td dose. Your account will be tagged with “UW HSIP- Subsequent Tdap/Td Vaccine Dose”.
- Annual Flu Vaccine: Your Med+Proctor account will become unverified near the week of 08/15/current year. Your account will be tagged with “UW HSIP – Required Influenza Vaccine”.
- Annual TBSS Form: Only required for positive TB screening students. Your Med+Proctor account will become unverified near the week of 12/01/current year. Your account will be tagged with “UW HSIP- Annual TBSS”.
- Annual TB Risk Assessment Form: Only required for School of Nursing students. Your Med+Proctor account will become unverified near the week of 12/01/current year. Your account will be tagged with “UW HSIP- Annual TB Risk Assessment”.
HSIP registration blocks are automated and are linked to your Med+Proctor account.
Initial Registration Block
When you are entered into an HSIP program as an incoming student, you will have an initial registration block. This block is to make sure you create your Med+Proctor account once you have been invited to do so.
For the registration block to be lifted, you will need to complete the Childhood Immunization Attestation Questionnaire. Once completed and the “UW HSIP – Required Childhood Immunization” tag has been removed by Med+Proctor, your registration block will be lifted in the next 10 minutes.
If you have completed the Childhood Immunization Attestation Questionnaire but your block has not been removed, please contact HSIP at myshots@uw.edu.
Winter Quarter Registration Block
After the initial registration block, incoming Summer and Autumn HSIP students will not have a registration block from HSIP until 10/15. As a reminder, please complete your requirements by the due date assigned by your program. Incoming students who are late with submitting their requirements and are not completed by 10/15 will have a registration block for Winter Quarter registration until compliant in Med+Proctor. Once you are marked as compliant or temporary compliant in Med+Proctor, your registration block will be lifted in the next 10 minutes.
Upcoming Requirement Registration Block
If you were previously compliant with your HSIP requirements but have an upcoming requirement due, you will have registration block placed if you do not complete the upcoming requirement:
- Temporary Compliance: You will have a registration block on the Temporary Compliance Due date listed in your Med+Proctor account.
- Expiring Tdap/Td Vaccine: You will have a registration block 30 days before the 10-year expiration date of your Tdap/Td dose.
- Annual Flu Vaccine: You will have a registration block on the due date of your program’s flu vaccine due date.
- Annual TBSS Form: Only required for positive TB screening students. You will have a registration block on 01/03.
- Annual TB Risk Assessment Form: Only required for School of Nursing students. You will have a registration block on 01/03.
You may receive the following emails from Med+Proctor while working on your requirements. Med+Proctor emails from admin@reminders.medproctor.com will be delivered to your UW email address. If your UW email address is not activated, please review the UW IT’s Email & Calendering page.
Invitation Email
If you have never made a Med+Proctor account with the UW, you will initially receive the following email from Med+Proctor when HSIP has configured your account as an HSIP account. If you had to use Med+Proctor for IVP requirements during undergrad, then you will not receive this email when your account is configured as an HSIP account.

If you do not receive this email, do not worry. Your program will send you instructions on how to access your Med+Proctor account. You can also use this tutorial to access and make sure your account is configured as an HSIP account. If have any questions or issues with your account, please reach out to HSIP at myshots@uw.edu.
Weekly Reminder Email
You will receive the following email when your Med+Proctor account is unverified every Saturday. The link in the email will direct you to the Status page of your account.

To check your messages in your Med+Proctor account, you can click on Messages and open the most recent message.

Account Verified Email
You will receive the following email once your Med+Proctor account has been marked a “Verified”. This means your account is EITHER fully compliant or you are temporarily compliant with your requirements. To check if you were given temporarily compliant status, please review the Document Review Status section of this tutorial.

You can print out an official immunization certificate that automatically generates with everything recorded for your requirements in your Med+Proctor account. To pull this record, you will need to go into the Status page of your Med+Proctor account and click on “Immunization Certificate.” You will be prompted to Save your immunization certificate into your device.

Once saved the immunization cerificate will download. Once downloaded, you can open and view your immunization certificate.
