Med+Proctor Tutorial

Submit your Immunization Documents

Submit your records for verification in Med+Proctor. Med+Proctor is a free service and payment is not required for non-Health Sciences students

All students are required to fulfill their immunization requirements. Listed below are instructions for general students (non-HSIP) and HSIP students. If you aren’t sure which you are, please see our home page or email us at immunity@uw.edu

Initial Log In 

To submit your documents through Med+Proctor, please sign-in through SSO using your UW NetID. It may require Two Factor Authentication (2FA). If you are unable to access your 2FA, please review the UW-IT’s 2FA information

You may receive an email welcoming you to use the Med+Proctor service if you are an incoming student. If you are an incoming student and you have not yet received this email, you are still welcome to submit your documents through Med+Proctor.
On your first time signing into Med+Proctor, you will need to confirm your date of birth in month/day/year (MM/DD/YEAR) format.

Below, the fields for First Name and Last Name have been greyed out, with black lines censoring the auto-filled name, but the field for Date Of Birth has been left empty. The First The field is framed in red, and on the top it says "Required". Under the field are the words "Format mm/dd/yyyy".

You will also need to agree to the End User License Agreement and write your full name to agree.

This is a popup screen. Agreement is bolded at the top left. Underneath is a line, and under that line starts the End User License Agreement. Under the block of text, there is a field that has "Enter your full name", and under this states "Enter your full name for use as an electronic signature."

HSIP Students will also need to agree to the HSIP Agreement as written on the HSIP website, answer the childhood immunization attestation, and respond to the TB Risk Assessment. These will be in your Questionnaire and Profile tabs. 

Med+Proctor Fees

General students (non-HSIP students): Once you have agreed, you will be asked if you would like to join MP Pro for benefits such as record retention and expedited review. The current price for MP Pro is a one-time $15 fee.  This is not required in order to process your documents. If you do not wish to pay for MP Pro, please click on the link under the list of benefits that says “No thank you. Submit your documents without MP Pro.”

Great for transfer of health information to other organizations. $2 Processing Fee added for Credit Card Payments. Purchasing MP Pro is not required to submit documentation."

Top left of the screenshot shows "Expediated Review Service". Under this is a faint line, and under that: "Wait time for review are currently around 72 hours. Med+Proctor offers an expedited option for you which guarantees your review in 24 hours for an additional service charge of $10. To opt in please click Yes below or continue to upload your document without our expedited review service." 

In a light blue font is a link: "Need a test, titer, or other lab results for your submission? Click here." 

Under another faint line on the bottom right is a red button that says "I don't want expedited service" and a green button that says "Yes! Expedite my review."

HSIP Students are required to pay the $24 or $29 fee, which is based on your program’s length and amount of Med+Proctor work. If you have any questions, please email us at myshots@uw.edu.

Document Submission

To submit your documents, click on the Documents page. For General students, if you do not have the Immunization Form or Exemption Form, you can also click to download these documents here. Choose a Document Type and upload your document. If you aren’t sure what document type you are submitting, please choose ‘Supporting Documentation’. Please note that if you do not press ‘Continue’, your uploads will be lost.

HSIP Students: Do not submit your exemption requests into Med+Proctor; email them directly to HSIP at myshots@uw.edu.

You can upload as many documents as required (see below). Please remember to include the identification page or section of each document; we need to see your full name and/or date of birth. PDF and JPEG file types are preferred.

If you need to submit more than one file, click on “Click here to submit another document” at the bottom of the screen. This will send you to the Status Page again, so you will need to continue to click Documents to submit another file. Submit as many files as you need for full information about your immunizations. 

General students will see the option to expediate documents; HSIP students will already have the expediated service.

Underneath this in blue is "Your record is being reviewed. Processing time is 48 hours" and "Med+Proctor will notify you via email when we approve your submission."

Under this is another link that states "Click here to submit another document."

Once you have submitted your documents, please wait 48-72 hours to receive a confirmation message from Med+Proctor in your UW email.

Document Review Results

If the submitted documents have satisfied the requirements, you will receive an email like the one below:

However, if your requirements were not satisfied by your documents, you will receive an email informing you of a secure message waiting for you on Med+Proctor. When you log into Med+Proctor again, you will see your messages as below.

On your Status page, you will also find your current tags, showing if you have not satisfied your requirements, and if there are any follow up steps, as well as reasons why it was rejected. You can click on the tag links to go to the Med+Proctor FAQ to see further information.
