Vaccination Events


Looking to get your vaccinations on campus? Join us at the following vaccine clinics!

For all events, please bring the following:

  • photo ID
  • UW student ID number
  • insurance card (or a copy or photo of the front and back of the card)

These vaccine clinics are not limited to students – anyone is allowed to join!

Also, please wear clothing with short sleeves or sleeves that can be pushed up.

HSIP students are invited to attend our vaccine clinic hosted by the Health Science Immunization Program and Safeway/Albertsons.

Date: September 4, 2025 from 11 AM to 4 PM

F-1 students who are covered under the ISHIP/Lifewise insurance can receive vaccinations at our vaccine clinic hosted by the Immunity Verification Program, Husky Health Center, and CIRCLE! All other students may attend other vaccine events.

You may add vaccinations at your appointment. Please make only one appointment, so that appointments remain available for other students.

NOTE: You must have the following forms prepared and completed before your vaccine appointment:   

You can do these during the MyChart eCheck-In, or print and bring the completed forms with you to the clinic. Otherwise, you will be asked to complete the forms at the time of your appointment. If you are 17 years old or younger, your parent/guardian must sign and complete the forms before the clinic, or they will need to give verbal consent over the phone. 

You can select multiple vaccinations to satisfy your requirement. 

Select a time that works best for you! Please note that the September 18th clinic will be at the HUB, North Ballroom, during the CIRCLE Post-Arrival Resource Fair.

In the Comments, please write your 7-digit Student ID number (e.g. 2500001). You do not need to write any other information.  

If you already have a MyChart account, click on log in and schedule. If not, please continue as guest, and click Schedule for yourself. Do not schedule for someone else.  

Fill in as much information as possible. If you live on campus, you can find your U.S. address here (Make sure you select your residence hall on the right). If you do not have a U.S. address yet, you may use the Husky Health address below: 

Please put in the email address you will most likely use. Do not use an email from @163.com, as these emails tend to be blocked.

If you do not have an American phone number yet, put the UW Immunity Verification Program’s number: (206) 616-4672).  

Put down your Ethnicity and Preferred Language.  

Please either write in “Lifewise” or select “Premera BlueCross” from the drop-down menu. It’s ok that you don’t know your insurance number yet; this information will come to you in early September. If you don’t know it by the time of the clinic, you will be asked to find it by the Lifewise table at the clinic on site.  

If you are the patient, and you are the one who purchased the insurance, click yes. If you are the patient, but you are under another person’s insurance (such as your parents’), then you are not the policy holder.  

You have made your appointment! Please keep the appointment time! 

You can make a MyChart account right now, which will help you schedule future appointments with Husky Health Center and download your medical records.  

You will receive an automatic email from MyChart confirming your appointment information. You will also be given an access code to create your MyChart account after you have made your appointment.  

If you need to cancel, you can click on the ‘Click here to reschedule’ and will be asked to put in your date of birth. You will then need to sign the agreement for using MyChart. 

Three days before your appointment, you will receive a request to “e-Check In”. This is not a “Check In”, this is a confirmation of your appointment. This request is not required, but it is useful to update your information, such as your address, phone number, sign any documents required before, and input your insurance information, which you should have access to. However, if you do not check in, you will need to update your information at the clinic.

Date: November 4, 2025 from 11 AM to 3 PM

Safeway/Albertson’s will be hosting the vaccine clinics on September 4th, September 30th, and October 9th in Seattle; October 2nd and 30th in Bothell; and October 7th and November 4th in Tacoma.

Following are instructions for signing up.

  1. Check that you have selected the right date and location when you sign up for an appointment and click “Proceed to Booking”.  

Health insurance information will be collected at the event. If you do not have your insurance information, you will be asked to pay the cost of the vaccine at the time of your appointment.   

  1. Fill out your date of birth and zip code. If you are new to the area or do not have an address yet, use “98195” for the UW Seattle campus zip code.  
  1. Select all the vaccines you would like to receive:  
  1. Select a time slot for your appointment. You can select between “Morning” and “Afternoon” to see more available times.  
  1. Fill out the required basic personal information and contact information. Your date of birth will be automatically filled in from when you entered it before. 
  1. If you do not have a local address, use the Immunity Verification Program address: “4060 E. Stevens Way NE, Seattle, WA, 98195”.  The zip code will be automatically filled in from what you entered on the first page. 
  1. Enter in your full name to acknowledge HIPAA privacy policies and complete the additional health information questions. HIPAA, or the Health Insurance Portability and Accountability Act of 1996, protects the privacy and sharing of patients’ health information. Click on “Please click to review” if you would like to read more. 

“Weight in lbs.” is only for patients who are children weighing less than 66 lbs or 29.9 kgs. If you weigh more than 66 lbs./29.9 kgs, leave this question blank.   

  1. If you have a Primary Care provider/doctor, click on “Yes.” Then, at minimum, fill out the name of your Primary Care provider/doctor before you click “Continue”. If you are new to the United States or do not have a provider or medical doctor in Washington State, click “No.” 
  1. The last step is to review all the details and information for your appointment and check that the information is accurate.  

Check all the boxes and enter your name to sign. Select “Self” under “relationship” if you are filling it out for yourself.  

Click “submit” to finish making your appointment.   

  1. You will see a page confirming your appointment request was successful. You will also be getting a confirmation email at the email address you provided.  
  1. You can download, print, and fill out the consent form to bring to your appointment. If you cannot do it ahead of time, you will have to fill out the consent form at the time of your appointment.  
  1. Check your email for confirmation.  

Please cancel or reschedule your appointment if you can no longer come to your original appointment time. This will give other students a chance to access this resource. Click on the link that says, “Need to Reschedule or Cancel?” 

  1. Once you click the link to reschedule or cancel, you will be taken to a webpage where you need to validate your information. Check your email again for the Authentication code.  
  1. Once you authenticate your information, it will display your appointment details, and you can click on the link at the bottom of the screen to either reschedule or cancel the appointment.  
  1. If you click “Cancel”, a window will pop up to ask you once more to confirm if you would like to cancel:  
  1. The website will then show confirmation that you have successfully cancelled your appointment and sent you another email confirmation:
