As a patient of Hall Health Center, you have rights and responsibilities.
Your Rights
- Access to care, treatment or accommodations.
- Access to an interpreter if you do not speak or understand English.
- To receive humane care and treatment.
- To receive accurate, current and complete information.
- To have no care provided to you without your consent.
- To be informed of any research aspect of your care and to refuse to participate.
- To receive a second opinion regarding diagnosis or treatment.
- To know who is counseling, caring for or treating you.
- To receive information regarding the scope and availability of services.
- To receive information regarding fees and payment policies for service.
- To voice your concerns or complaints about your health care.
- To have information regarding advance directives.
- To have your health care information protected to ensure privacy and confidentiality.
- To access to your medical records through MyChart or by completing and submitting a release of information form.
Your Responsibilities
- Provide complete and accurate health information to those caring for you.
- Participate in discussions and ask questions about your care.
- Satisfy out-of-pocket costs associated with your care or make payment arrangements in a timely manner.
- Do not give medications prescribed for you to others.
- Show respect to Hall Health staff and patients.
- Do not lend your Husky Card or insurance card to others.
- Keep your appointments OR notify Hall Health at least 24 hours in advance if you need to cancel or reschedule. If you do not contact us to cancel, you may be charged a $40 no-show fee. For mental health therapy appointments, you may be charged a $20 late cancellation fee if you do not provide 24 hours notice in canceling your appointments.
- Watch your children to prevent safety issues and avoid disturbing other patients and families.
