Husky Health Center Joins UW Medicine Financial Assistance Policy

As of 7/1/2025, Husky Health Clinic will be a part of the UW Medicine financial assistance policy.

Effective July 1, 2025, Husky Health will be a part of the UW Medicine financial assistance policy for professional services with dates of service on or after July 1, 2025. UW Medicine provides financial assistance for any patient/guarantor whose gross family income is up to 400% of the Federal Poverty Level (FPL) and adjusted for family size after any third-party coverage has been exhausted.

0% – 300% of the FPL for a 100% financial assistance discount for facility and/or professional services at Airlift Northwest, Harborview Medical Center, UW Medical Center, UW Physicians, UW Medicine Primary Care, Husky Health Center, and Valley Medical Center.

301% – 350% of the FPL for a 75% financial assistance discount for facility services only with discharge dates on or after July 1, 2022, at Harborview Medical Center, UW Medical Center, and Valley Medical Center.

351% – 400% of the FPL for a 50% financial assistance discount for facility services only with discharge dates on or after July 1, 2022, at Harborview Medical Center, UW Medical Center, and Valley Medical Center.

How to apply:

To determine your eligibility for financial assistance, you will need to complete an application and provide certain supporting information to verify your household income. A patient, family member or anyone involved in the patient’s care, including your UW Medicine healthcare provider, may apply for financial assistance.

You can apply online through MyChart.  MyChart offers the ability to track your application status.  To apply by mail, fax or in person you can submit your application through UW Medical Center or Harborview Medical Center.

Husky Health Center Closed July 4

Following the UW Holiday Calendar, Husky Health is closed on Friday, July 4th, 2025 in observance of Independence Day.

HHC will resume regular hours and operation on Monday, July 7th. Husky Health Center is closed on every official University of Washington Holiday.

Med+Proctor Tutorial

Submit your records for verification in Med+Proctor. Med+Proctor is a free service and payment is not required for non-Health Sciences students

All students are required to fulfill their immunization requirements. Listed below are instructions for general students (non-HSIP) and HSIP students. If you aren’t sure which you are, please see our home page or email us at immunity@uw.edu

Initial Log In 

To submit your documents through Med+Proctor, please sign-in through SSO using your UW NetID. It may require Two Factor Authentication (2FA). If you are unable to access your 2FA, please review the UW-IT’s 2FA information

You may receive an email welcoming you to use the Med+Proctor service if you are an incoming student. If you are an incoming student and you have not yet received this email, you are still welcome to submit your documents through Med+Proctor.
On your first time signing into Med+Proctor, you will need to confirm your date of birth in month/day/year (MM/DD/YEAR) format.

You will also need to agree to the End User License Agreement and write your full name to agree.

HSIP Students will also need to agree to the HSIP Agreement as written on the HSIP website, answer the childhood immunization attestation, and respond to the TB Risk Assessment. These will be in your Questionnaire and Profile tabs. 

Med+Proctor Fees

General students (non-HSIP students): Once you have agreed, you will be asked if you would like to join MP Pro for benefits such as record retention and expedited review. The current price for MP Pro is a one-time $15 fee.  This is not required in order to process your documents. If you do not wish to pay for MP Pro, please click on the link under the list of benefits that says “No thank you. Submit your documents without MP Pro.”

HSIP Students are required to pay the $24 or $29 fee, which is based on your program’s length and amount of Med+Proctor work. If you have any questions, please email us at myshots@uw.edu.

Document Submission

To submit your documents, click on the Documents page. For General students, if you do not have the Immunization Form or Exemption Form, you can also click to download these documents here. Choose a Document Type and upload your document. If you aren’t sure what document type you are submitting, please choose ‘Supporting Documentation’. Please note that if you do not press ‘Continue’, your uploads will be lost.

HSIP Students: Do not submit your exemption requests into Med+Proctor; email them directly to HSIP at myshots@uw.edu.

You can upload as many documents as required (see below). Please remember to include the identification page or section of each document; we need to see your full name and/or date of birth. PDF and JPEG file types are preferred.

If you need to submit more than one file, click on “Click here to submit another document” at the bottom of the screen. This will send you to the Status Page again, so you will need to continue to click Documents to submit another file. Submit as many files as you need for full information about your immunizations. 

General students will see the option to expediate documents; HSIP students will already have the expediated service.

Once you have submitted your documents, please wait 48-72 hours to receive a confirmation message from Med+Proctor in your UW email.

If the submitted documents have satisfied the requirements, you will receive an email like the one below:

However, if your requirements were not satisfied by your documents, you will receive an email informing you of a secure message waiting for you on Med+Proctor. When you log into Med+Proctor again, you will see your messages as below.

On your Status page, you will also find your current tags, showing if you have not satisfied your requirements, and if there are any follow up steps, as well as reasons why it was rejected. You can click on the tag links to go to the Med+Proctor FAQ to see further information.

Sexual Health

Enjoying a healthy, pleasurable sex life, whether with a partner or on your own, is an important part of well-being. Husky Health offers an array of sexual health services to help you protect yourself from STIs and unintended pregnancy. We also offer pregnancy testing, options counseling, medication abortion, abortion referral, and prenatal care. 

Free safer sex supplies 

UW Seattle students can drop by Husky Health Room 101 during our hours of operation to pick up 10 supplies per week.

Sexual Health Services at Husky Health 

Many sexual health services are available via telemedicine. Call us at 206-685-1011 to schedule: 

  • Confidential testing for sexually transmitted infections, including chlamydia, gonorrhea, herpes, syphilis and HIV 
  • Treatment for STIs, included partner treatment 
  • PrEP (pre-exposure prophylaxis), a prescription medication for people who are at risk of HIV that prevents HIV transmission 
  • Pregnancy testing, options counseling 
  • Medication abortion 
  • Prenatal care 
  • A wide variety of birth control methods, including the birth control pill, patch, ring, shot, IUDs, implant, and diaphragm 
  • IUD and implant (Nexplanon) insertion and removal 
  • Cervical cancer screening (Pap and HPV testing) 

Emergency contraception 

There are three types of Emergency Contraception (EC) and all are available at Husky Health: Paragard copper IUD, Ella and Plan B. Plan B is also available over the counter at the Rubenstein Pharmacy, most other pharmacies, and a vending machine on campus at Odegaard Library.

HPV vaccination 

Husky Health offers HPV vaccination to UW Seattle students and established Husky Health patients. Please call us at 206-685-1011 to schedule. 

Bring your insurance card and student ID and check-in on the first floor. 

Vaccines that are recommended for you based on your age and assigned sex are fully covered by most US insurance plans when you visit an in-network provider. Learn more about health insurance and cost

STI testing and treatment (including PrEP) 

To schedule an appointment for STI testing, call us at 206-685-1011. Be sure to let us know when you call if are experiencing symptoms. 

STI testing needs vary from person to person based on age, assigned sex and risk factors. Unfortunately, in most cases, it’s not possible for Husky Health staff to tell you in advance of an STI testing visit what the out-of-pocket cost will be. The cost will depend on your health insurance benefits and network, the complexity of the visit and which lab tests are included. You can learn more about your health insurance benefits here. See the FAQ below for more information. 

Birth control methods 

  • If you are a current UW Seattle student, you can talk with our Patient Healthcare Advocate (PHA) about contraceptive options at no cost. Call us at 206-685-1011 to schedule a virtual, telephone or in-person visit. The PHA cannot order lab tests, perform exams or write prescriptions. 
  • For prescription birth control, including to schedule a pre-IUD or pre-Nexplanon consultation, call us at 206-685-1011. 

Pregnancy testing, options counseling, and medication abortion  

Husky Health offers pregnancy testing, options counseling, medication abortion, and prenatal care.  Our Patient Healthcare Advocate and consulting nurses are trained to provide accurate, non-judgmental information about all pregnancy options. We are here to support you with education, guidance, and care as you explore your choices. 

Support for survivors of sexual & relationship violence  

If you have experienced or are experiencing any sort of unwanted sexual encounter, and would like to discuss support options and resources, you can speak to a confidential advocate through the Survivor Support and Advocacy Program at UW LiveWell. There is no cost to speak with an advocate. The Confidential Advocates can also can discuss resources and options if you are experiencing relationship violence, stalking, or sexual harassment. Please email them to set up an appointment at lwadvoc@uw.edu. The Confidential Advocates work with all UW Seattle students and all tri-campus faculty and staff, as well as the Medical Center employees.

What is the cost of STI testing or birth control? Will it be covered by insurance?  

The out-of-pocket cost of care depends on your insurance benefits and network. All US insurance plans are required to cover the full cost of any birth control method, as long as you visit an in-network provider. The cost of STI testing depends on whether or not you have symptoms, the tests you are seeking and your insurance benefits. 

If you are a current UW Seattle student, you are eligible for one subsidized visit (face-to-face interaction) with a Husky Health provider (medical doctor or nurse practitioner) per quarter for a problem or concern. However, you are responsible for any ancillary services, like lab tests, imaging and medications. We will bill your health insurance unless told otherwise, in which case you will be charged for any services not paid for through your subsidized benefits as a UW student.

Is this service confidential?  

Your health information cannot be shared without your express permission with anyone other than the providers of your medical care. There are two exceptions to this rule: billing health insurance for STI testing or birth control and reportable STIs. 

If you have health insurance through a parent, guardian, or spouse, it is possible that the primary subscriber on the insurance could receive mail from your insurer that includes information about health care services you received, including STI testing and birth control. Paying out-of-pocket and not billing your insurance is the only way to be certain of confidentiality. 

Some STIs are reportable by law. Positive chlamydia, gonorrhea and syphilis results must be reported to the local health department to make sure those affected are treated. If you do have a positive result on one of these tests, your information will still be protected, and you would only be contacted if you did not complete treatment. Positive HIV results are also reported to the local health department, but anonymous HIV testing is available in Seattle (though not at Husky Health Center) if this is a concern.

To speak with an insurance navigator, someone who can help you get enrolled and answer your questions, students in Washington State can reach out to the agents on the Washington Health Plan Finder website. You can also find support by enrolling through insurance brokers.

In Washington State, if you are under 19 years old, you can receive free immunizations through the Childhood Vaccine Program. If you are 19 or older and are uninsured or underinsured, you may qualify for the Adult Vaccine Program

Bloodborne Pathogens

Had a needlestick or blood exposure accident? Don’t panic!

While you are studying and learning how to treat patients and our community, you will need to know what to do in the event of bloodborne or body fluid exposure. Follow the steps below immediately within 24 hours after the incident.

If you have a needlestick injury or similar bloodborne exposure, the best thing to do is to act now.  

  1. Perform First Aid: 
    Wash the wound thoroughly for 15 minutes with warm water and sudsing soap. If eye exposure, use emergency station to flush eyes for 15 minutes while holding eyes open.
  2. Get Medical Help:  
    • Notify your supervisor.
    • If the patient consents, be sure to receive the patient’s name, date of birth, and MRN number for later follow-up.
    • If at a UW Medical site, go to the Employee Health Center for immediate assistance. If the exposure occurred at a non-UW Medical site, go to the nearest Emergency Room. Do not go to the non-UW Employee Health Center, as you are not considered an employee of the site, and will confuse insurance payouts later.
  3. Complete the BBP Redcap Survey within 24 hours to Redcap first, and THEN UW Online Accident Report (OARS). This will assist in further medical treatment and follow-up care, and help prevent other incidents for your classmates.

Treatment & Coverage of Costs Related to Blood & Body Fluid Exposures

Exposure Treatment & Coverage of Costs at a Non-UW Medicine Site

Students on rotation at a non-UW Medical site who have a blood or body fluid exposure during a clinical or practicum experience should be seen at the nearest ER and use their personal insurance for treatment. HSIP will pay or reimburse any uncovered costs after the student’s insurance is billed first.

  • Health science students are not employees while they are engaging in education-related activities at clinical sites and are, therefore, not covered by workers’ compensation at either the clinical site or UW.  Treatment for exposures is NOT considered an on-the-job injury since students are NOT employees at clinical training sites.  
  • Follow-up care after an initial evaluation for an exposure should be arranged through the UW Campus Employee Health Center (EHC), if indicated to do so. Contact UW EHC at emphlth@uw.edu or call 206.685.1026.
  • If the exposure occurs outside the Seattle area, please contact HSIP at myshots@uw.edu for assistance with arranging follow-up care, if indicated.  
  • HSIP assistance with medical expenses is only for BBP exposure; it does not include other types of injuries or illnesses that may be related to your clinical or practicum experience.
  • HSIP does not cover after-insurance reimbursement for exposures related to international travel. Students are strongly encouraged to obtain and carefully review the terms of supplemental travel insurance prior to each trip.
  • Students are eligible for the reimbursement described in this section only if they are enrolled and have paid the HSIP fee.

If your insurance does not cover all the costs associated with post-exposure care, or if you have questions about reimbursement for expenses related to a BBP exposure, needlestick or sharps injury, please contact HSIP staff by email at myshots@uw.edu and note in the email subject line: “Reimbursement Question.” Alternatively, you can leave a voice mail message at 206.616.9074.

Exposure Treatment & Coverage of Costs at a UW Medicine Site

Students on rotation at a UW Medicine site (UW Medical Center, UW Northwest Medical Center, Harborview Medical Center, UW Dental Clinic, or other UW Medicine site) who have a blood or body fluid exposure during a clinical or practicum experience should be seen immediately at those sites’ corresponding Employee Health Center during regular business hours Monday – Friday between 8:00 a.m. and 4:00 p.m.  

  • Follow-up care after an initial evaluation for an exposure should be arranged through the UW Campus Employee Health Center (EHC), if indicated to do so. Contact UW EHC at emphlth@uw.edu or call 206.685.1026.
  • Students will not be billed for costs of exposure care performed at any UW Medicine Employee Health Centers. 
  • After hours students at a UW Medicine site should be seen at the nearest ER and use their personal insurance for treatment.  HSIP will pay or reimburse any uncovered costs after the student’s insurance is billed first.  Health science students are not employees while they are engaging in education-related activities at clinical sites and are, therefore, not covered by workers’ compensation at either the clinical site or UW.  Treatment for exposures is NOT considered an on-the-job injury since students are NOT employees at clinical training sites.

Health Science Immunization Requirements

The Health Science Immunization Program (HSIP) ensures students enrolled in health science academic programs comply with the requirements detailed in the University’s affiliation agreements with clinical and practicum training sites. These affiliation agreements require students to complete and document immunizations and tuberculosis (TB) screening as recommended by the Centers for Disease Control and Prevention for healthcare personnel. The requirements reduce the risk of exposure to communicable diseases for students and their patients/clients.  Students are required to comply with updated CDC and HSIP requirements upon enrollment and continue to meet ongoing requirements until completion of their degree program, even if those requirements change during a student’s enrollment. It is the student’s responsibility to meet any requirements of a training site that may differ from those requested by HSIP.

Due to the nature of health science training programs, exemptions are granted in alignment with CDC recommendations for documented medical conditions for which there is a vaccine contraindication, HSIP policy, and University policy and practice only. Please refer to the Vaccine Exemptions section below.

Health Science Immunization Program

Students enrolling in these UW health science programs that require clinical or practicum training to complete the degree should expect to receive notification from academic program staff of the HSIP requirements:

Please note:

  • Pre-health science students do not participate in HSIP.
  • Students under 22 years old must also complete the Meningitis ACWY requirement in the UW General Immunity Verification Program.

HSIP contracts with two vendors to track immunization and TB testing compliance for the incoming programs. Each health science school or program sends instructions to accepted students for creating their vendor account, including deadlines for meeting HSIP requirements. Some programs require additional accounts for background checks and drug screenings which are not related to the HSIP accounts. Each account type must be set up separately. There is a one-time fee to set up the vendor account paid directly to the vendor when creating the account, which will be listed in the instructions email sent by your programs. Please do not make an account before your program sends you instructions to do so, as this can affect your account.

Beginning with incoming Autumn 2025 students, most incoming programs will be accessing Med+Proctor to submit their documents. HSIP also plans to transfer current students into Med+Proctor, starting in Winter 2026.

Once you enter a participating health science school or program, you will pay the HSIP fee to cover the cost of administering the Health Science Immunization Program. The amount of the fee is subject to change on a yearly basis and is attached to your tuition account for as long as you remain in your UW health science school or program.

We recommend students learn what health services are offered at the student health center on their “home” campus and in the community. For students who need initial or ongoing services in the WWAMI region (Washington, Wyoming, Alaska, Montana, and Idaho), please review our Locations page.

By accepting enrollment into a program/school that requires participation in the UW Health Sciences Immunization Program (HSIP), students agree to meet immunization and TB screening requirements in accordance with CDC guidelines for healthcare personnel and as outlined by HSIP. This includes a requirement that students be vaccinated against COVID-19. Due to the nature of health sciences training programs, exemptions are rarely granted and only in alignment with CDC recommendations for documented medical conditions for which there is a vaccine contraindication, HSIP policy, and University policy and practice.

HSIP follows the Centers for Disease Control and Prevention’s (CDC’s) Guidelines for Healthcare Workers, which may change during a student’s enrollment. Students are required to comply with updated CDC and HSIP requirements upon enrollment and continue to meet ongoing requirements until completion of their degree program, even if those requirements change during a student’s enrollment. It is the student’s responsibility to meet any requirements of a training site that may differ from those requested by HSIP at the time of enrollment.

HSIP may discuss, share, and communicate students’ compliance status and related information with their health sciences school/program and practicum/clinical training sites prior to and during placements. This includes any or all information provided to meet entry requirements and any annual or other required updates. In addition, HSIP may share student immunization status with the University of Washington’s Employee Health Center in the event of an exposure to a bloodborne pathogen.

This agreement must be signed in CastleBranch and/or Med+Proctor.

These due dates have been chosen by your program. Students accepted late into their program have 10 days after their acceptance to submit their exemption requests.

Immunization Requirements

The Childhood Immunization is an attestation confirming the receipt of the DTap (or DTP/DT) and Polio vaccine series. Documentation is not required. On Med+Proctor and CastleBranch, these questionnaires will populate once you have made an account.

Respond YES to the attestation if you received the usual childhood immunizations for DTap (or DTP/DT) 3-doses and Polio (OPV/IPV) 3-doses.

Respond NO and contact HSIP at myshots@uw.edu if you did NOT receive DTap (or DTP/DT) or Polio (OVP/IPV). HSIP will provide directions for the adult make-up schedule.

Submit one of the following:

  • a Tetanus, Diphtheria, & Pertussis (Tdap) vaccination administered in the past 10 years

OR

  • Tetanus & Diphtheria (Td) vaccine administered in the past 10 years AND documentation of one Tdap vaccine administered on or after 6/1/2005.

Renewal will be set 1 month prior to the 10 year expiration. If your Tdap/Td expires while you are a student, you will be required to receive another Tdap/Td.

Students must satisfy the MMR requirement by showing proof of:

  • two MMR (Measles-Mumps-Rubella) vaccines that must be taken after the student’s first birthday and taken at least 28 days apart from other live vaccines, such as Varicella.

OR

  • positive Measles (Rubeola), Mumps, and Rubella IgG titers, which are blood tests to measure immunity.

To fulfill the requirements, MMR vaccines and/or titers must meet the following conditions:

  • MMR vaccines must be taken no earlier than a student’s 1st birthday
  • MMR vaccines must be taken no earlier than 28 days between all live vaccines, such as MMR or Varicella. MMR and Varicella vaccines taken on the same day is acceptable.
  • MMR titers must be at least 28 days after a recent MMR vaccine.
  • History of Measles, Mumps, and/or Rubella is not an acceptable document. If you previously had Measles, Mumps, or Rubella, please take a titer test to show your immunity.
  • If the live vaccines (MMR and Varicella) are taken within 28 days, both doses will be rejected. If the MMR and Varicella doses are taken within 28 days of a TB screening, all doses and tests will be rejected. Please receive your MMR and Varicella vaccines at least 28 days before your Tuberculosis screening.

Students must satisfy the Varicella requirement by showing proof of:

  • two Varicella vaccines that must be taken after the student’s first birthday and taken at least 28 days apart from other live vaccines, such as MMR.

OR

  • positive Varicella IgG titers, which are blood tests to measure immunity.

To fulfill the requirements, Varicella vaccines and/or titers must meet the following conditions:

  • Varicella vaccines must be taken no earlier than a student’s 1st birthday
  • Varicella vaccines must be taken no earlier than 28 days between all live vaccines, such as MMR or Varicella. MMR and Varicella vaccines taken on the same day is acceptable.
  • Varicella titers must be at least 28 days after a recent Varicella vaccine.
  • History of Varicella is not an acceptable document. If you previously had Varicella, please take a titer test to show your immunity.
  • If the live vaccines (MMR and Varicella) are taken within 28 days, both doses will be rejected. If the MMR and Varicella doses are taken within 28 days of a TB screening, all doses and tests will be rejected. Please receive your MMR and Varicella vaccines at least 28 days before your Tuberculosis screening.

Students must satisfy the Hepatitis B requirement by showing proof of:

  • Documentation of 3 Doses Hepatitis B (Engerix-B, Recombivax HB, or other HepB vaccine) OR 2 doses of Heplisav-B

AND

  • positive Hepatitis B Quantitative Surface Antibody Titer

Step 1: Hepatitis B Vaccine Series

Students should try to obtain their childhood immunization records. If you cannot obtain documentation of a previous vaccine series you do not have proof of your Hepatitis B vaccines, you are required to repeat the vaccine series.

If your vaccine series is in process, submit where you are in the series. New alerts will be created for you to complete the vaccine series and titer. A titer will NOT be accepted until after a complete HepB vaccine series has been submitted.

Acceptable Vaccine Doses and Schedules

Your Hepatitis B vaccine doses need to meet the minimal intervals to be accepted. If you have a dose that does not meet the minimal intervals, then that dose is considered invalid, and you will need to complete a new Hep B dose to replace the invalid dose.

  • Energix-B, PreHevbrio, or Recombivax-HB (3-dose series): Doses are given at respectively 0, 1, and 6 months (*minimum dosing intervals are acceptable: dose 1 to dose 2: 4 weeks / dose 2 to dose 3: 8 weeks / dose 1 to dose 3: 16 weeks)
  • Twinrix (HepA-HepB 3-dose series): Doses are given at respectively 0, 1, and 6 months (*minimum dosing intervals: dose 1 to dose 2: 4 weeks / dose 2 to dose 3: 5 months).
  • Accelerated Twinrix dosing schedule (4 doses; dose 1 to dose 2: 7 days / dose 2 to dose 3: 21-30 days / dose 3 to last dose: 12 months) is accepted. However, students who choose this option should remember that 1 month after the last dose, they will still need to receive the titer to prove immunity.
  • Heplisav-B (2-dose series): Doses are given at least 4 weeks apart.

Step 2: Quantitative Hepatitis B Surface Antibody

After completion of a full Hep B vaccine series, you need to complete a Quantitative Hepatitis B Surface Antibody Titer (HBsAb-QN).

We require a positive Hepatitis B titer result in a laboratory report with the quantitative numeric interpretive reference range. An immunization summary or Employee Health titer is not acceptable. A qualitative titer test that only says positive with no reference range is not acceptable. Please note that we do not require a core or Antigen test unless you are a non-responder or have a chronic infection.

Negative Hepatitis B Titer

If your titer is negative or equivocal:

  1. Submit the negative titer to your immunization portal.
  2. Receive and submit one additional vaccine dose (Heplisav-B highly preferred) immediately.
  3. Get a repeat titer 1 month after the booster dose, and submit the report. If it is positive, you have satisfied the requirement.
  4. If the second titer is negative, you will need to receive the remaining Hepatitis B vaccine series. If you chose the 2 dose series, you will need to complete the remaining 1 dose. If you chose the 3 dose series, you will need to complete the remaining 2 doses.
  5. Get a final titer 1 month after the last vaccine dose in the second series. If it is positive, you have satisfied the requirement.
  6. If this last titer is negative, please email HSIP at myshots@uw.edu.

Hepatitis B Non-Responder

An individual is considered a Hepatitis B non-responder once they have completed two documented Hepatitis B vaccine series followed by a negative/equivocal HBsAb titer. Hepatitis B non-responders must complete a Hepatitis B Surface Antigen Titer (HBsAg). If the antigen titer is negative, then your provider needs to complete the HSIP Hepatitis B Non-Responder Counseling Form. Please email myshots@uw.edu to request this form.

For a non-responder, you must complete the following tasks:

  • Obtain your most recent Hepatitis B Surface Antigen (HBsAg) titer lab report.
  • Please have your provider complete the HSIP Non-Responder Counseling Form.
    Once you have completed the form, please submit them directly to HSIP myshots@uw.edu.

Please note that your status as a Non-Responder will be shared with your program. Students are expected to inform their practicum of their Non-Responder status as well.

Hepatitis B Chronic/Acute/Past Infection

Individuals with a known history of chronic or acute Hepatitis B Infection should complete the following steps; please request forms from the HSIP at myshots@uw.edu. If your antigen titer is positive, then your provider needs to complete the HSIP Acute-Chronic Infection Counseling Form. Please email myshots@uw.edu to request this form.

  • Obtain your most recent Hepatitis B Surface Antigen (HBsAg) titer lab report.
  • Please have your provider complete the HSIP Acute-Chronic Infection Counseling Form.

Once you have completed the forms, please submit them directly to HSIP at myshots@uw.edu.

Please note that your status as a Non-Responder will be shared with your program. Students are expected to inform their practicum of their Non-Responder status as well.

A baseline individual TB risk assessment must be completed upon entry into your program. The risk assessment is required in addition to the TB Screening requirement and can be useful in interpreting TB test results.

Please complete the HSIP TB Risk Assessment Form fully and provide yes/no answers for each question. A baseline individual TB risk assessment must be completed upon entry into your program. Students must satisfy the Tuberculosis requirement by showing a Tuberculosis screening from this calendar year (1/1/current year):

  • IGRA blood test (either a QuantiFERON Gold or T-Spot lab test)
    • Preferred option for all students. Students with a history of receiving the BCG vaccine against TB should get the IGRA test to avoid a false positive PPD skin test.
  • TWO separate TB skin tests/PPDs
    • First PPD placement and reading 48-72 hours later equals ONE PPD skin test. A SECOND PPD skin test must be placed between 1-3 weeks after the first PPD is read. The HSIP TB PPD Skin Test Form is recommended to ensure all required documentation is included.​
    • Required: Full name, date each PPD skin test was placed, date each PPD skin test was read, and results documented as a measurement in millimeters.
    • “Self-reads” are not acceptable.

Complete Tuberculosis (TB) screening (IGRA or TB PPD skin tests) before receiving a live virus vaccine (MMR, Varicella); otherwise wait 28 days after the live vaccine before beginning TB screening. Same day IGRA blood draw and administration of a live vaccine is ok.

Positive TB Screening

If you have a positive TB screening, you are required to must complete and submit the following:

  1. Documentation/proof of positive screening (one of the following):
    • Positive IGRA result (either a QuantiFERON Gold or T-Spot lab test) or
      PPD result 10 mm or greater or
    • HSIP Positive TB Screening Form completed by a healthcare provider
  2. Chest x-ray radiology report completed after positive TB screening AND after January 1st of your program entry year.
    • Note: If there is documentation of a completed course of prophylactic antibiotic therapy, including medication name and dates of administration is provided, then an older chest x-ray may be accepted.
  3. HSIP’s TB Symptom Survey (TBSS) Form completed by the student initially.

Students with a positive TB submission will be required to fulfill the TB Symptom Survey (TBSS) requirement annually between December 20 – December 31. Submit an updated HSIP TB Symptom Survey (TBSS) Form to meet the TBSS subsequent action. You DO NOT need a new TB test or x-ray.

NOTE TO STUDENTS: In the event of either a TB exposure, a practicum site requiring an updated TB screening, or for those who are working in a high-risk area, additional TB screening may be required. Please contact HSIP at myshots@uw.edu to have a new action created to upload any additional TB screenings.

Students in the Medical Laboratory Sciences/Laboratory Medicine program are required to receive Color Vision screening from the HSIP nurse and staff. This will be arranged for you; do not schedule your own color vision screening. If you do have color vision deficiencies, your status will be recorded to your instructors for accessibility and alternate identifiers.

Students must satisfy the Influenza requirement by showing a flu vaccine taken from the upcoming/current flu vaccine season. For Summer/Autumn 2025 and Winter/Spring 2026 admits, students are required to provide an updated 2025-2026 formulation of influenza vaccine, available in August. All continuing students must also complete the 2025-2026 formulation of influenza vaccine.

Only Influenza vaccines administered after 08/01 of the current flu season will be accepted. Previous flu vaccines are not accepted.

All flu vaccines must show the location a student received the vaccine, such as the pharmacy or clinic they received the vaccine.

Students must satisfy the COVID-19 requirement by showing a COVID-19 vaccine taken from the upcoming/current COVID-19 vaccine season. For Summer/Autumn 2025 and Winter/Spring 2026 admits, students are required to provide an updated 2025-2026 formulation of COVID-19 vaccine, available in September 2025. All continuing students must also complete the 2025-2026 formulation of COVID-19 vaccine.

Students are required to provide one of the following:

  • An updated 2025-2026 formulation mRNA vaccine (Pfizer/Comirnaty or Moderna/Spikevax), OR
  • An updated 2025-2026 formulation Novavax vaccine PLUS documentation of at least one additional COVID-19 vaccine (additional vaccine must be approved for use by the World Health Organization).

Only COVID-19 vaccines administered after 08/01 of the current COVID-19 season will be accepted. Previous COVID-19 vaccines are not accepted.

All COVID-19 vaccines must show the COVID-19 manufacturer that the student received. COVID-19 vaccines may be administered without regard to timing with other routine vaccines and TB screening required for HSIP students.

Tested Positive for COVID-19?

Students may request a 90-day extension for an updated COVID-19 vaccine following a recent COVID-19 infection.

Complete the COVID-19 Vaccination Extension Request Form and return it to HSIP at myshots@uw.edu.

NOTE: Students will have a 10-day grace period to get vaccinated once the extension expires.

Every program sets the deadlines that all students need to meet in order to register and attend practicum.

Exemption requests for required Health Science Immunization Program (HSIP) vaccines must be requested upon admission into your school/program by the date listed below. Exemption requests for the autumn vaccine requirements (influenza and COVID-19 vaccines) are also due by this date. Students accepting admission after the due date will have 10 days from their acceptance date to submit an exemption request.

Medical Exemptions

Vaccine medical exemption requests must be submitted by the exemption due date based on your admission quarter into your HSIP participating program. Students with new or worsening health concerns may submit a medical exemption request at any time.

Medical exemption requests are reviewed by the HSIP medical director for medical validity. Additional testing, medical consultation, and/or documentation may be required to receive an exemption.

Instructions for submitting a vaccine medical exemption request: Complete the Health Science Immunization Program (HSIP) Participant Medical Exemption Request for Required Vaccines form and submit it to HSIP at myshots@uw.edu. Do not upload a request into CastleBranch or Med+Proctor.

Please note: Antibody titers will be required for MMR, Varicella, and Hepatitis B vaccine exemptions to verify immunity status even if a vaccine exemption is approved. TB screening is not eligible for exemption.

Religious Exemptions

Vaccine religious exemption requests must be submitted by the exemption due date based on your admission quarter into your HSIP participating program. Students may submit a vaccine religious exemption request after the due date; however, programs may not be able to accommodate late requests.

Religious accommodations for required HSIP vaccines must be approved by the student’s school/program.

Instructions for submitting a vaccine religious accommodation request: Complete the Health Science Immunization Program (HSIP) Participant Religious Accommodation Request for Required Vaccines form, Sections #1 to #4, and submit it to HSIP at myshots@uw.edu. Do not upload a request into CastleBranch or Med+Proctor. Do not sign Section #6 until the exemption has been approved. The HSIP manager will forward your request to your program’s designated program executive for review.

Please note: Antibody titers will be required for MMR, Varicella, and Hepatitis B vaccine exemptions to verify immunity status even if a vaccine exemption is approved. TB screening is not eligible for exemption.

Non-Religious Declination

Due to the nature of health science training programs, non-religious vaccine declinations are not accommodated by health science programs participating in the Health Science Immunization Program (HSIP). Please contact your program directly if you have questions regarding personal non-religious declinations.

Exception: Students enrolled in HSIP participating School of Public Health MPH programs may request a non-religious vaccine declination; however, accommodations of an approved declination will require students to complete practicum/capstones in a non-healthcare setting. Contact the HSIP at myshots@uw.edu for information.

All required documentation components must be included in your vaccine record submission:

  • Full name. If the name on your record does not match your current legal name you will be required to verify your name change through submission of a legal name change document (court issued record, marriage certificate, etc.).
  • Name of vaccine administered and full date of administration (mm/day/year).
  • Documents must be from a verifiable medical source (medical record, clinic or provider signature/stamp, or certified state immunization registry).
  • Handwritten K-12 records signed by parents are not acceptable.
  • When submitting a vaccine booklet the following must be included:
    • The front page of the booklet with full name must be included in the same file upload as the vaccine documentation.
  • Each vaccine dose listed must include a provider signature/initials or facility name.
  • Pharmacy printouts, provider/clinic-verified childhood immunization information, and state registry records are all acceptable proof of immunization.
  • The military immunization form DD 2766 is acceptable only for immunizations; all titer and lab tests must be a laboratory test result document.
  • All laboratory reports must have the student’s full name and the specimen date of collection (blood draw date).
  • MMR & Varicella IgG Titers – results may be qualitative (e.g. positive/negative)
  • Quantitative Hepatitis B Surface Antibody Titer – results should be quantitative (numeric value). A qualitative result (positive/negative or reactive/nonreactive) is acceptable only when a numeric interpretation reference range is also included (e.g. immunity ≥10 mIU/mL).
  • IGRA Blood Test – Acceptable IGRA blood tests are either a QuantiFERON-TB Gold test or a T-Spot test. Qualitative results (positive/negative) accepted. Numeric values preferred but not required.
  • The Result Trends document is not accepted.
  • If your records are not in English, please make sure your name is in English on your documents. Your records should be easy to read and tell what titers, immunizations, and screenings you received. English translation is welcome but not required.

JPEG and PDF file types are preferred. Please make sure your documents are not password-protected, and that your images are clear and readable before uploading. You can upload as many documents as needed into Med+Proctor.

Printing Documents from a Patient Portal

If you have access to your records through a patient portal, you may be able to access your report by going to the lab tab details and clicking the printer icon in the upper right-hand corner. Make sure the header is set to be included, then save a copy of the print screen as a PDF.

The Result Trends documents are not acceptable; this type of report does not include the source of the record and therefore is not verifiable.

This document should have your full name, test name, collection date, results, and show the source of the record.

If a lab was completed through LabCorp or Quest Diagnostics, you can find the lab report through their sites.

You are considered temporarily compliant if you are between vaccine doses, or waiting to complete your Quantitative Hepatitis B Surface Antibody Titer.

In CastleBranch, the requirement will show as incomplete in your CastleBranch account with a future due date. You are considered temporarily compliant while completing the vaccine series as long each new due date is met, and documentation has been been submitted and accepted into CastleBranch.

Submit each dose as it is received and a new action for the next dose/titer will be created with a new due date. The due date listed in CastleBranch is the date you should get the next vaccine dose in the series. For the HepB titer, the due date represents the date you should plan to get the titer drawn.

How to Fulfill Your Immunization Requirements

Students should check with their primary care provider for the students’ vaccinations, or any organization where they received vaccinations.

Students can also check with their state or country department of health. For vaccinations taken in Washington state, students can use the website MyIR Mobile. For students who had vaccinations in other states, please find your state’s immunization database on this State Immunization Registries document. For students who received your vaccinations outside of the United States, please receive your immunization documents from your primary physician.

Suggested places to check for old immunization records:

  • Immunization records from previous clinics or providers
  • Pharmacy prescription documents for vaccinations
  • Previous employers* (employee health records) including military*College or other post-secondary institutions* (K-12 records that are not provider-verified are not accepted)
  • Department of Health
  • Provider-verified immunization booklet
  • A facility stamp or provider signature is required next to each recorded vaccine dose. Unverified records should be taken to students’ provider or local public health clinic and documented in an official medical record or state immunization registry.

The UW HSIP is not responsible for students’ insurance or covering vaccines or titer tests, although we do recommend having insurance.

If a student is looking for locations on or near the three UW campuses that offer vaccinations or titer tests, the student can look through the following links to find a suitable option. The student should contact clinics directly regarding their sliding scale or financial assistance options, as well as their insurance. When a student requests financial assistance, they should be ready with medical and tax information.

If a student is interested in having insurance but does not know how to get started, the student can make use of Husky Health’s Insurance and Cost explanation, which explains insurance options and how to apply.

To speak with an insurance navigator, someone who can help you get enrolled and answer your questions, students can reach out to the agents on the Washington Health Plan Finder website. They can also find support by enrolling through insurance brokers.

Seattle students can call Husky Health Center at 206-597-5244 to make an appointment with an insurance navigator; Tacoma students can call Tacoma Office of Student Advocacy and Support at 253-692-5934. There are no current options for Bothell students specifically, but students are welcome to research through the WA Health Plan Finder.

If a student has insurance through their job they are planning to leave to study full-time, we recommend doing all and any requirements while you are currently insured.

How do I submit my documents with Med+Proctor?

Med+Proctor is our new vendor for immunization verification which we started to use for the HSIP students in March 2025. Many programs have already transitioned to Med+Proctor; if you are not sure where to go, please email myshots@uw.edu. To login, you will need to have your netID and be a student capable of enrolling. If you have not yet confirmed your status as a student, please reach out to your advisor to confirm.

All students who were previous general students with the IVP requirements will be asked to submit the further requirements of HSIP. If you have been ‘verified’ without earlier documents, please contact myshots@uw.edu.

JPEG and PDF file types are preferred. Please make sure your documents are not password-protected, and that your images are clear and readable before uploading. You can submit as many documents as you required into Med+Proctor.

Rejected Submission

Submissions through Med+Proctor take 48-72 hours to be processed. Students will receive an email from Med+Proctor if their documents have been accepted or rejected. If the documents have been rejected, students can log into Med+Proctor and review the rejection information.

If you have not yet fulfilled your immunization requirements, there will be a notice in your MyUW account. You will also be able to see the exact reason for rejection on your Med+Proctor Status page. Clicking on the reason will give you a full explanation hosted on Med+Proctor’s website.

How do I submit my documents with CastleBranch?

Tips to ensure successful uploading of your documents into CastleBranch:

  • Chrome is the preferred web browser.
  • Convert photo files into PDF files to decrease the file size. Files must be 5 MB or smaller.
  • Upload files from your computer or flash drive using the Browse link at the bottom of the to-do action (fax/mail is not recommended).
  • You must upload into each to-do action in your CastleBranch account.
  • Documents saved in the Document Center are not accessible by administrators.
  • If a submission is listed as Pending for more than 48 hours, try submitting it again.

Rejected Submissions

Items rejected in CastleBranch are missing required documentation components. You may review the rejection reason by clicking on the + icon next to the requirement name in your CastleBranch to-do list. The rejection reason will be at the top of the dropdown.

Fixing your rejected submission:

  • Review the full requirement instructions in your CastleBranch to-do action.
  • Review the required documentation components to ensure your submission meets the criteria (e.g., your full name must be on the document).
  • Contact HSIP at myshots@uw.edu for assistance with your requirements. Do not contact the CastleBranch vendor.

How to schedule an appointment at Husky Health Center 

Eligibility

Many of our services for new patients are reserved exclusively for University of Washington Seattle Campus students. Our scheduling team will confirm your eligibility to use Husky Health Center when you schedule your first appointment.  

New Patients

  • Set up an account on MyChart. If you have already set up an account on UW Medicine MyChart, skip this step.  
  • If you would like to confirm that Husky Health Center is in-network, contact your insurer or visit our insurance information page.  
  • Make an appointment for medical care by calling 206-685-1011. 

Returning Patients

  • Schedule an appointment by calling 206-685-1011 or online using MyChart. 
  • Contact your primary care provider directly or request new prescriptions for previously prescribed medication using MyChart. Contact your pharmacy directly (if refills remain).

Billing Information at Husky Health

Bill in the Mail? 

Husky Health bills health insurance for most services. You may receive a bill in the mail associated with your care. When you come to Husky Health for care, there are different entities within our building that may bill for services. 

If you receive a letter that says “Explanation of Benefits” from your insurance company, it is not a bill. If you do have an out-of-pocket cost, you will receive a bill from UW Physicians, the entity that manages billing for provider time, the UW Department of Laboratory Medicine, for lab services, or Husky Health Center. Often, a bill will arrive 2-4 weeks after an explanation of benefits letter. 

You may receive an explanation of benefits but not ultimately be billed if you are a current student and you received subsidized services. Read more about subsidized services

Collections 

If you have a bill that has not been paid for 120 days or longer, it will be sent to collections. To pay your bill, you will need to contact State Collection Service at 1-800-477-7474. See the FAQ below for information about how to get a registration hold lifted. 

All non-student patients are charged a fee for service. We will directly bill your insurance plan whenever possible. All patients are responsible for co-payments and non-covered services. You can learn more about your health insurance by following the instructions here

Please note that Husky Health Center is not able to see new non-student patients with the following insurance plans, or any other ACN plan

  • Apple Health (Medicaid) – any managed care plan (Molina, Coordinated Care, Amerigroup, CHPW, United)

Read more about insurance plans at Husky Health here. If you do not have health insurance, you will be asked to make a deposit at the time of your appointment.  The amount of the deposit will be specified when you schedule an appointment.  This may or may not cover the total cost of the visit. 

Please note that some Husky Health services are not available to non-students even with contracted insurance, including: 

  • Services through our same-day care clinic 
  • In-person drop-in medical advice through our Consulting Nurses 
  • The following services are not available to non-students without first establishing care at Husky Health by scheduling an appointment with a medical provider: 
  • Immunizations 
  • Lab tests 

Charity Care 

Husky Health does not receive government funding and cannot provide reduced-fee services of any kind beyond the subsidized services offered to currently matriculated UW-Seattle students

If you are uninsured or underinsured and seeking affordable care, it may be beneficial to establish care with a Community Health Center

UW Bothell and UW Tacoma Students 

Bothell and Tacoma students are welcome to visit Husky Health for their care, including for immunizations and blood tests associated with the UW General Immunization Requirement or Health Sciences Immunization Program

However, before you seek care at Husky Health, review the following points: 

  • We are not able to accept new non-UW-Seattle students with Medicaid (Apple Health). If you have Apple Health, we’d suggest visiting your managed care plan’s website to find an alternative contracted provider.  
  • You will need to establish care at Husky Health by seeing a medical provider (MD or NP) prior to obtaining immunizations, blood tests, or medical advice from a nurse. 
  • Some services are not available to you, including care through our same-day clinic, and in-person medical advice from a nurse or a Patient Healthcare Advocate 

Confidentiality, Insurance, and Deposits 

If you previously allowed us to bill your insurance, and now ask us to not bill it due to confidentiality concerns, we will ask you for a $200 deposit at check-in. You will be reimbursed if the cost of your care is less than $200. 

This may happen if a student is concerned about their family finding out about services they sought (i.e., STI testing, birth control). Husky Health would never release this information without your explicit consent, but insurance companies follow different rules. 

If you have health insurance through a family member, it is possible that the primary subscriber could find out which services you received if they were to receive a statement in the mail from the insurance company. However, you can call your insurance company and ask them to send any statements to you directly. Be sure that the billing address Husky Health has on file for you is your own and not your family’s.
