We will process your immunizations within 24 hours of our receiving them. However, if you are in a hurry or want to make sure we see your confirmation, please forward the email informing you of the changes to us with your student ID number, like below.
We accept:
Every document must have the student’s name and date of birth, vaccination date and brand, and must be verified by either a medical professional or an immunization report. It must be a legal, official report.
Check with your primary care provider to see if they have the records of your vaccinations, or any organization where you received vaccinations.
You can also check with the state or country department of health. For vaccinations taken in Washington state, you can use the website MyIR Mobile.
Also, if you have received vaccines in the State of Washington, the Washington State Immunization Information System (WAIIS) may contain your immunization records. You may authorize UW to query WAIIS for these records to assist you in satisfying your immunization requirement. Please update your Consent for Immunization Record Query for us to be able to look up your WAIIS record. Once you confirm your authorization, you can either forward us your confirmation email from the Office of the Registrar or wait 24-48 hours for us to receive it automatically and look up your records. You will receive an email letting you know whether the immunizations listed in your WAIIS record were sufficient to fulfill your immunity requirements.
There are many options for students who are worried about the cost of vaccines or bloodwork.
When you submit your records in the portal, you will receive an automatic email to your UW email address informing you that you have submitted your documents, and that your documents will be processed in 1-2 days, with longer wait times depending on how many submissions we receive. Please be patient and check your emails.
You will receive an email to your UW email address once your submission has been reviewed, informing you of its status. If it has been accepted, you will receive an email informing you that you are no longer blocked from registration. If it has been rejected, or does not fulfill all your immunity requirements, you will receive an email informing you why it was rejected and how to fix the problem.
Students who have not met the Measles, Mumps, and Meningitis (MMM) requirements will not be able to register for their second quarter.
While we understand that a registration block is a significant disruption for students, our priority is protecting the UW community from infectious diseases like Measles, Mumps, and Meningitis.
If you find that you have a registration block, review the specifications of the requirements listed here and correct your mistake. Check your email to see if we have informed you why a submission was rejected.

Please check your UW email account. After you submit your immunization records, you will receive a message there letting you know what was accepted, and noting any problems.
Incoming students will receive periodic reminders to their UW email, reminding them to satisfy their requirements before their Submission and Registration dates. If you have not yet fulfilled your immunization requirements, there will be a notice in your MyUW account.

If you are in the following Health Sciences programs, you need to meet the Health Sciences Immunization Program requirements, which are administered by UW Environmental Health & Safety. You do not need to satisfy the general UW requirement, unless you are under 22 years old, in which case you must satisfy the Meningitis ACWY requirement. If you are under the age of 22 as of the first day of the quarter and entering one of the programs listed above, you will need to provide proof of Meningococcal ACWY immunization in addition to your HSIP requirements:

We may still have the records that you sent to us. The University of Washington retains records for 6 years after your last contact with the University. However, if we have the written information on your student records, we can send you that information. Email your full name, date of birth, student ID number and netID with the records request, and we will send your records via an encrypted email for security purposes.
You still need to satisfy your Measles, Mumps, and (if under age 22) Meningitis requirements. Please submit your vaccines here.
You are welcome to submit them without a translation. We are able to read records in several languages. If we cannot, we will reach out to you for clarification or assistance.

Hall Health is able to accept walk-ins at this time. If you are looking to receive immunizations, please contact 206-616-2495 to make an appointment or call ahead. Hall Health staff are not able to address registration holds or uploaded immunization documents sent through the portal. Managing this requirement is an administrative responsibility led by the Immunity Verification Program.
Do not go to the Office of the University Registrar or campus registration office for help releasing your registration hold regarding your immunization requirements, as they will not be able to help you, and will redirect you to this site as well. If one clinic or pharmacy does not have the vaccine or appointments, you’re welcome to find another.
You can request Measles and Mumps titers (blood immunity tests), to see if you have immunity to these diseases. You can request these from your primary care physician, most clinics, or you can go to Hall Health Clinic in Seattle.
We accept only positive test results, not negative or equivocal.
If either or both of your Measles and Mumps titer test results are negative or equivocal, you should take an MMR vaccine, and then wait at least 28 days and take a second MMR vaccine or a second titer test. If you have a second titer test and again receive a result that is not positive, you will need to take another MMR vaccine.
Rubeola is the medical word for Measles. Rubella is the medical word for German Measles. We require the Rubeola titer test, but not the Rubella titer test. Please check before you submit your titer test results that you are sending the Rubeola (Measles) and Mumps results.

Yes, you need a second dose of the Meningitis ACWY vaccine. The vaccine should be taken after students’ 16th birthday. We will accept a student’s Meningitis ACWY vaccine if they received it at age 15, but no younger. Students under the age of 16 are not required to fulfill the requirement until their 16th birthday.
Yes, you are allowed to wait until you arrive on campus. You should be able to receive the vaccine at any of the locations mentioned on the sidebar of this page. However, please pay attention to the deadline to turn in proof of your vaccination.
No. You need to have a Meningitis ACWY vaccine to prevent the A, C, W, and Y variants of Meningitis.
We cannot accept polysaccharides, as they are less effective against Meningitis than conjugate vaccines.

You can request a temporary extension by calling or emailing us and explaining why you were not able to fulfill the requirement. We can offer 40 days as an extension, or until the next registration period, whichever comes soonest. We cannot give second extensions, as students will have been emailed several times about the requirement.

To obtain an exemption to the Measles, Mumps, and/or Meningitis ACWY immunity requirement, please download and print the form below, and have it signed by a health care professional.
Your exemption request will be reviewed within 3-5 days of receipt.
