Students may request an exemption based on medical decisions or religious beliefs. Please see sections below and choose your specific exemption carefully. Note that each exemption form has its own submission portal and process.
The UW does allow students to pursue permanent or temporary exemptions to all or part of the UW Measles, Mumps, Meningitis and Covid-19 Immunity Verification Requirement. Exemptions must be related to a documented medical issue or religious objections and must be reviewed by a health professional. You will need to complete this process even if you previously had an immunization exemption at another school, or had one as a child and are now an adult.
If there is an outbreak of Measles, Mumps, or Meningitis, the student may be asked to leave the campus during the outbreak.

Request Exemptions for Measles, Mumps, and/or Meningitis ACWY (MMM)
To obtain an exemption request form and to begin the process, please download and print, and have it signed by a health care professional. For religious exemptions, the medical professional will only offer the possible risks and benefits of the vaccines. Medical exemption requests must be signed by a health care professional. To request a temporary medical exemption, please write the date by which you will be able to receive the vaccines.
Your exemption request will be reviewed within 1-2 weeks of your submission. You will receive an email notifying you of the decision. If you are requesting a temporary medical exemption, you will receive another email informing you of the expiration date.
Request Exemptions for COVID-19
To obtain an exemption form and to begin the process, please download and print, and have it signed by a health care professional. For religious exemption requests, the medical professional will only offer the possible risks and benefits of the vaccines. Your religious exemption request will be reviewed within 1-2 weeks of your submission.
Medical exemption requests require a healthcare professional’s signature and the medical reason for the exemption. These typically take longer for review.
To submit your form, make sure you log into the Docusign portal using your UW email address/netID. If you do not, it will be rejected, and will not release your registration hold. You must use your NetID for the exemption forms to be eligible for acceptance.

Students who are temporarily unable to complete a full primary COVID-19 vaccine series for reasons beyond their control (such as living in a country without access to WHO-approved vaccines) may request a deadline extension. Extensions will only be provided to allow sufficient time to become fully vaccinated upon arrival on campus, or as appropriate to each individual situation. Extension/exemption requests related to a medical condition require submission of a COVID-19 vaccination medical exemption request form, not a deadline extension request.
