Students may request an exemption based on medical decisions or religious beliefs. Please see sections below and choose your specific exemption carefully. Note that each exemption form has its own submission portal and process.
The UW does allow students to pursue permanent or temporary exemptions to all or part of the UW Measles, Mumps, and Meningitis Immunity Verification Requirement. Exemptions must be related to a documented medical issue or religious objections and must be reviewed by a health professional. You will need to complete this process even if you previously had an immunization exemption at another school, or had one as a child and are now an adult.
If there is an outbreak of Measles, Mumps, or Meningitis, the student may be asked to leave the campus during the outbreak.

Request Exemptions for Measles, Mumps, and/or Meningitis ACWY (MMM)
To obtain an exemption to the MMM immunity requirement, please download and print the form below, and have it signed by a health care professional. For religious exemptions, the medical professional only needs to discuss the possible risks and benefits of the vaccines with you. In the case of a medical exemption, the healthcare professional will take your individual medical consideration into account, and if you need only a temporary medical exemption, should write the date by which you will be able to receive the vaccines.
Your exemption request will be reviewed within 1-2 weeks of your submission. You will receive an email notifying you of the decision. If you are requesting a temporary medical exemption, you will receive another email informing you of the expiration date.
